1.0 PURPOSE
1.1 The objective of this procedure is to ensure effective swift execution of emergency operations by the Dermaga
Oil & Gas Sdn Bhd (DOGSB) emergency response to achieve the following:-
1.1.1 The safety and health of all personnel;
1.1.2 Preservation of environment, facilities and reputation of Client‟s / Owner‟s and Contractor;
1.1.3 Prevent losses and protection of investments;
1.1.4 To ensure effective coordination between DOGSB‟s and Client‟s / Owner‟s emergency response
teams
2.0 GENERAL
2.1 When an emergency situation arises, the DOGSB‟s (Project Management Team) shall manage and control
activities that are within their means;
2.2 The DOGSB‟s however shall work with BOMBA or outside emergency team (e.g. Client‟s / Owner‟s response
team) and obtain additional resources and facilities;
2.3 The DOGSB‟s site emergency team headed by Emergency Team Commander (ERT) shall activate the first or
initial response;
2.4 The ERT commander shall mobilize personnel under his charge comprising of leader, fire fighter, first aid BA
(Breathing Apparatus) handler (if required) and medical personnel to arrive at the scene as fast as possible;
2.5 Immediate action shall be taken to save lives and protect property. Upon arrival of the Client‟s / Owner‟s Duty
Manager or his representative, the ERT commander shall handover command and take all necessary
instructions from him;
2.6 Instructions can be activating evacuation from site, search and rescue (SAR) of missing persons or medical
evacuation of personnel.
3.0 PROCEDURES
3.1 An orderly evacuation depends on both an early warning and employee awareness of the proper procedures
to follow. The procedures below apply to all section (department):-
3.1.1 Each section/department shall establish procedures to be followed regarding the evacuation of
buildings in emergencies;
3.1.2 Where possible, key emergency instruction shall also be highlighted and posted at prominent places
of the workplace;
3.1.3 Every section/department has been installed with Break Glass or Alarm System (Office Building) to
alert employees to the need for evacuation;
3.1.4 Concise emergency instructions shall be posted at strategic locations throughout the premises,
including the floor plan drawn to indicate the emergency exits and evacuation instructions;
3.1.5 Evacuation drills shall be held at least once a year.

3.1.6 Emergency exits and routes leading to them shall be clearly identified by sign. Current standards on
construction, dimensions, lighting, and number of exits required by Uniform Building by Law & Fire
Services Act of Malaysia shall apply.
3.1.7 As applicable, location procedures should include the following minimum personnel / employee
action after the alarm has sounded.
3.2 Employees:-
3.2.1 Most importantly don't panic;
3.2.2 Take whatever immediate steps that is necessary and feasible to minimize any hazard in leaving
the work area unattended. Quickly evacuate and don't go back in the building;
3.2.3 Assemble at a pre-determined safe location (Assembly Point) for attendance check;
3.2.4 Do not re-enter building until the "All Clear” signal sounds or similar verbal instructions are given by
responsible authority.
3.3 Supervisors:-
3.3.1 Direct the evacuation of your area and account for personnel;
3.3.2 Advise the responding authority (e g. Company‟s Emergency Response Squad, Bomba and etc.) of
the situation and warn of potentially hazardous conditions.
4.0 SCOPE
4.1 The scope of this procedure shall: -
4.1.1 Be applicable to all emergencies within DOGSB‟s operations;
4.1.2 Provide guidelines for DOGSB‟s personnel and emergency response team should any emergency
occurs;
4.1.3 Provide guidelines for notification, coordination between Client‟s / Owner‟s and DOGSB‟s emergency
response teams;
4.1.4 Provide guidelines on response activities and support for all emergency team members to response
in tandem during emergency situation.

8.0 COMMUNICATION
8.1 Effective communications are essential during any emergency situation and the importance of maintaining the
uninterrupted communication with site involved with the emergency cannot be emphasized enough.
8.2 Without such communication, Emergency Response team will not be in the position to give full support to the
site emergency team. The following general rules shall be adhered during any emergency situation:-
8.2.1 ERT Commander shall carry mobile phone and radio (if available) at all times;
8.2.2 All communications except those directly associated with the emergency should cease;
8.2.3 Unauthorized use of portable radios is forbidden as this will create confusion;
8.2.4 Messages regarding the emergency shall whenever possible be by telephone or fax to ensure some
form of confidentiality. This is of particular importance with regard to the names of the casualties;
8.2.5 Emergency calls should be brief and accurate;

8.2.6 Telephone messages regarding victims including casualties should always be followed up by fax
signed by responsible person or by e-mail if fax facility is not available;
8.2.7 Keep an accurate log of all calls made concerning all emergencies;
8.2.8 All external transmissions by telephone or fax must cease unless authorized by the person in charge;
8.2.9 Radio communications (if available) regarding any distress or emergency situation shall be on the
channel being set for emergency only.

15.0 FIRE ALARM SYSTEM (IN THE BUILDING / COMPLEX PROVIDED WITH FIRE ALARM SYSTEM)
15.1 Fire Alarm System is used to warn employees of emergency conditions and to trigger an orderly evacuation of
the buildings. Such system also provides the means to activate fire control equipment and notify BOMBA and
other emergency services.
15.2 Fire Alarm System can be either manual or manual-automatic in operation. Manual-automatic systems detect
a pre-determined condition and activate the alarm system automatically, in addition to being manually
operated by an employee at the location.

17.0 REPORT
17.1 All fire incidents in the plant shall be investigated and reported. The purpose of this report is to find out what
caused the fire and what corrective action will be taken to prevent from re-occurrence;
17.2 In addition, all firefighting equipment used to put out the fire shall be reported using the Report on Used
Firefighting Equipment.
b) Respective head of sub-contractors with the help of concerned supervisor shall ensure their
workers are totally evacuated and check the area under his responsibilities.
18.4 Immediate Action In Emergency Situations: -
18.4.1 If danger is present, look after yourself first. If you become a casualty, the situation worsens;
18.4.2 Preferably remove danger from casualties. If impossible, remove the casualties from danger;
18.4.3 Electrical contact - break the contact, if possible by switching off the current first;
18.4.4 Poisonous fumes or escaping gas - try to cut off the source but if impossible, remove the casualty;
18.4.5 Fire and falling rock - remove the casualty. However, for casualties with spinal injuries, they should
not be moved if at all possible;
18.4.6 Road traffic accident - arrange for someone to divert the traffic, do not remove the casualty;
18.4.7 Send for help at once, especially if there is more than one casualty. Remember any telephone will
do - Dial emergency services.
18.4.8 Make use of other people by getting them to:-
a) Control traffic;
b) Summon professional help, giving clear instructions about nature of emergency, precise location
and number or casualties;
c) Disperse casual onlookers.
18.5 Advice On Calling For Assistance: -
18.5.1 If possible, call for assistance yourself. If it means leaving a casualty who needs your attention, get
someone else to do it.
18.5.2 If a second person sends for help, make sure they give the right message. Make them repeat it or, if
possible, write it down.
18.5.3 Dial 999 for emergency assistance.
18.5.4 Refer to the internal Telephone Speed Dial Directory to contact the respective Function Head in
case of after office hour emergency.
18.5.5 Ask for necessary emergency service - Ambulance, Bomba or Police.
18.5.6 When you get through, give the telephone number so they can ring back if you get cut off. Having
given the telephone number, state: -
a) Location of accident – state where it happened;
b) Details of injuries - as much as possible;
c) If it is a medical condition, for example - heart attack, stroke, childbirth - say so. You need only
telephone one service - they will notify the others, if necessary.
18.6 Procedures For Specific Situations: -
18.6.1 Fire And Explosion:-
a) Except where fires result from large scale explosions, small fires must be checked as soon as
they start as the first five minutes are considered vital.

b) Only through good housekeeping, prompt action by employee, proper firefighting equipment
storage and common sense precaution will prevent a small fire from becoming a disaster.
18.6.2 Flood:-
a) In the event of extremely heavy rain resulting to "flash" floods, bursting of storage tank (e.g.
Bomba Water Tank, Sprinkler or Hydrant Water Tank, water piping for cooling of machinery and
etc.) The Following procedures should be followed:-
i. Turn off water at source if possible.
ii. Isolate building premises by closing doors.
iii. Isolate all electrical supply on machinery and equipment.
iv. Raise up all electrical appliance above ground and salvage as much as possible all raw
materials and components.
v. If available, restrict the flow of water from entering the building / room using sandbags,
cotton rags and etc.
vi. Contact Mechanical & Electrical Department for assistance.
vii. Consider evacuation:
Partial evacuation of building employees by word of mouth.
Building evacuation - depress Break Glass Alarm.
18.6.3 Power Failure: -
a) Contact Project Manager / Project Engineer to determine cause of failure and notify to Safety
Personnel.
b) Consider evacuation of building by word of mouth. Switch off all electrical power before leaving
working place.
18.6.4 Hazardous Materials And Chemical Spills: -
a) Should a disaster occurs, fence the spilled area with barricade tape and block with sand, earth
or any absorbent to prevent liquid from entering sewers drain or low areas.
b) Eliminate sources of ignition; prevent additional discharge or material, if possible to do so
without hazard. Wear Personal Protective Equipment (PPE) when handling spillage e.g. safety
glasses, rubber glove or apron.
18.6.5 Poisonous Gas Or Smoke: -
a) The use of a lifeline is a valuable safety precaution and must be used.
b) In the case of gas:-
i. Before entering the area breathe in and out several times.
ii. Then take a deep breath and hold it.
iii. Go in and remove the casualty to safety.
iv. If it is not possible to immediately rescue the casualty, endeavor to cut off the source of
danger by turning off the gas or switching off the engine and obtain a full supply of fresh air
by opening doors and windows (except in the case of fire).

18.7.4 Responsibilities of Fire & Emergency Operation Coordinator (FEOC) - Safety Section
a) The FEOC Coordinator shall ensure that all Action Plan activities are conducted is a safe manner
as possible under the circumstances.
b) Obtain necessary safety equipment and facilities supply from logistics
c) Maintained all requires records and documentation to support the history of the emergency or
disaster. Document the following info:-
i. Message received
ii. Action Taken
iii. Decision justification and documentation
iv. Requests filled
v. Monitor drills, exercise and emergency response.
vi. Identify and mitigate safety hazards and situations.
vii. Stop or rectify all unsafe operations
viii. Keep the ERTC advised of your status and activity and on any problem areas that now
need or will require solutions.
18.7.5 Responsibilities of Damage Control Team
a) First Aid Team:-
i. Provision of emergency medical response, first aid and counseling.
ii. Muster all first aid team and report to FEOC during emergency
iii. Ensure all medical equipment and facilities (e.g. stretcher, first aid box etc) are available and
sufficient enough for operation.
iv. Take instruction from FEOC or obtain information through walkie-talkie (if available)
v. Call ambulance or transport seriously injured personal to the hospital by designated transport.
vi. Only trained First Aider is authorized to move injured personnel.
vii. Setup First Aid Area (Staging Point) for equipment and supplies in safe place with access to
emergency vehicles.
viii. During emergency case, medical assistant (First Aid Team Leader) shall stay at safe area
while search and rescue team doing the rescue operation and over to First Aider at the safe
area for further consideration or treatment.
b) Firefighting & Rescue Team: -
i. Mustering at Emergency Command Center and reported to FEOC and ensure firefighting
gears are available.
ii. Ensure all equipment for firefighting & rescue are available (.e.g; fire hose, fire extinguisher,
etc)
iii. Report to FEOC:-

a) Any event or condition which whilst not resulting in injury or illness to a persons or loss of, or
damage to equipment, plant or property is recognized as having potential to cause of property
damage.

19.5.4 Any accident which result in the loss of life or severe bodily injury to any persons and prevent him
from working for more than 4 (four) days or serious damage to any machinery, the company
(Project Director/Manager/Safety Manager) shall report to Department Occupational Safety &
Health (DOSH) by the quickest available means.
19.5.5 All occupational disease or injury or illness also shall report to Department Occupational Safety &
Health (DOSH) by the quickest available means.
19.5.6 Any accident which resulting in the loss of life(s) shall be reporting to the nearest Police Station.
19.5.7 All accident and fires (including near misses) must be investigates by the “Accident Investigation
Team” team comprising of: -
a) Project Director/manager
b) Department Head or Manager Concern

19.5.8 Department Head or Manager shall review the accidents or fire report and must adopt precautions
to prevent recurrence of similar accident, near misses or fire.
19.5.9 In event of serious body injury, fatality or dangerous occurrence, “Board Of Inquiry” will be setup
comprising of: -
a) General Manager
b) All Department Head and Manger
c) Department Concern
d) Safety & Health Manger
19.5.10 The immediate supervisor or superior shall complete the “Accident / Near Misses / Fire Notification
Report Form and submit it to Safety Department (Safety & Health Manager) immediately.
19.5.11 Upon receive the report Safety & Health Officer together with supervisor concern will conduct
investigation to identify the finding or cases related to the accident, near misses or fire including
corrective action to be taken.
19.5.12 After the investigation, Safety & Health Officer will write the accident, near misses or fire report.
The copy of report will be submitted to:-
a) General Manager
b) All Department Head and Manager
c) Department Concern
d) Safety & Health Officer
19.5.13 Safety Section shall process all necessary documents for Department Occupational Safety &
Health (DOSH), SOCSO as required for submission.

20.5 All the facts and background information relating to the incident: -
20.5.1 If loss of life is the result of an accident occurring during operations on an installation such operations
shall be suspended immediately.
20.5.2 The site of the accident shall be left undisturbed and sealed until the relevant authorities give
approval to resume operations.
20.5.3 If leaving the site undisturbed creates a hazard to personnel or property, steps should be taken to
ensure the site is made safe. However, prior to disturbing the site, a series of photos should be taken
for evidence.
20.6 When fatality or death occurs in the worksite/working place, the body shall be not removed and the area
declared “Out Of Bonds”. The body shall cover with pieces of cloth or other means.
20.7 The security guard will control and stand by at the scene of accident to control the situations.
20.8 Safety Section (Safety & Health Manager / Officer) will make report on the incident to nearest Police
Department after knowledge of management.
20.9 Security guard on duty has to ensure that no outsider, visitors, vehicle or reporter are allowed to enter
incident area/premises except for Police, Government Officer (who have jurisdiction in investigation) and
Medical Team.
20.10 Only Project Director or company-authorized representative is allowed to make press release on the incident
(after approval from management).
20.11 The Safety Section (Safety & Health Manager / Officer) will assist the Police Department, Department
Occupational Safety & Health (DOSH) or other authorities‟ government department who have jurisdiction in
their investigation.
20.12 Admin Department (admin Officer) will be identified and contact the victim‟s parents (families).
20.13 All matters pertaining to the coroner inquiry will handle by the Personnel & Admin and Safety Section
(Manager).
21.3 DOGSB‟s Project Management Team (Project Director / Manager) shall endorse the form “JKKP 6 or JKKP
7” before sending to Department Occupational Safety & Health (DOSH) for their action and investigation.
21.4 The supervisor shall furnish all details on the accident.
21.5 The Company shall undertake to carry out all remedial measures and procedures recommended by
Department Occupational Safety & Health (DOSH) as soon as possible.
24 INTERPRETATION
24.1 Serious Body Injury: -
24.1.1 Emasculation.
24.1.2 Permanent privation of the hearing or either ear.
24.1.3 Privation of any members of body or joint.
24.1.4 Destruction or permanent impairing of the powers of any members or joint.
24.1.5 Destruction or permanent impairing of the powers of any members or joint.
24.1.6 Permanent disfiguration of the heads or face.
24.1.7 Permanent privation of the sight of either eye.
24.1.8 Fracture or dislocation of a bone.

24.1.9 Any hurt which may endanger life, or which causes the sufferer to be during the space of the
twenty days (20), in severed bodily pains, or unable to follow his/her ordinary pursuits.

24.2 Fire (Type Of Fire): -
24.2.1 Class “A”
a) Fire involving solid materials (normally of an organic nature) in which combustion occurs with
the formation of glowing members (e.g. wood, paper, nature fiber etc.)
24.2.2 Class “B”
a) Fire involved liquids or liquefiable solids either soluble or insoluble in water (e.g. petrol, diesel,
methanol, thinner etc.)
24.2.3 Class “C”
a) Fire involving gases, liquefied gases in the form of liquid gas spillage or gas leak (e.g.
methane, propane, butane, acetylene etc.)
24.2.4 Class “D”: -
a) Fire involving metal such as magnesium, aluminum etc.
24.3 Dangerous Occurrences: -
24.3.1 Collapse or failure of crane, derrick, winch, hoist or other appliance used raising or lowering
persons or goods or overturning of crane;
24.3.2 Bursting or revolving vessel, grinding stone, grinding wheel move by mechanical power;
24.3.3 Explosion or failure of structure effecting the safety or strength of an unfired pressure vessel;
24.3.4 Any unintended collapse or partial collapse of any building or structure under construction,
alteration or demolition involving a fall of more than 5 tons of materials or any floor or wall of any
building being used as a place of work;
24.3.5 The uncontrolled accident release of the escape of any substance from any apparatus, equipment,
pipe line, process plant, storage tank, which may cause death, or major injuries other than
damage to health of nay person and environment.
