AFFIDAVIT OF SELF-ADJUDICATION
OF ESTATE OF THE LATE
ERLINDA L. ALVAREZ

I, BERNARDO V. ALVAREZ, Filipino, of legal age, widower, and a resident of Blk.1

Lot 5, Pkg. 1, LFUP,Gumaok, San Jose Del Monte City, Bulacan, after having been duly sworn
in accordance with law, hereby depose and state:

1. That I am the husband and the only compulsory heir of the late ERLINDA L.
ALVAREZ, who died in San Jose Del Monte City, Bulacan on July 27, 1999;

2. That my said deceased wife died intestate, without any Last Will and Testament and
without leaving any debts whatsoever;

3. That my said deceased wife left a parcel of land with residential house located at
Liberty Farms Upgrading Project, Gaya-gaya ( now Gumaok), San Jose Del Monte City,
Bulacan, which we own in common as our conjugal property, and covered by Transfer
Certificate of Title No.T-394549 of the Registry of Deeds Meycauayan Branch, and more
particularly described as follows:

“ A parcel of land (Lot 5-A of the subd. plan Psd-03-096835 being a portion of
Lot 5, blk. 1, Psd-80422 LRC Rec. No. ), situated in the Bo. of Gaya-gaya, Mun. of San
Jose Del Monte, Prov. of Bulacan. Bounded on the W., along line 1-2 by Lot 4; blk 1; on
the N., along line 2-3 by Rd. lot 1 both Psd-80422; on the E., along line 3-4 by Lot 5-B of
the subd. plan; on the S., along line 4-1 by lot 8, blk. 1 Psd-80422. Beginning at a pt.
marked “1” on plan being N. 26 deg. 20’E., 2331.85 from LM 10 Tala Est. thence N. 00
deg. 17’E., 10.29 m. to pt. 4; S. 89 deg. 42’W., 4.61 m. to pt. of beginning; containing an
area of FORTY SIX (46) Sq. M. All points ref. to are indicated on the plan and are
marked on the grd. by PS cyl. conc. mons. 15 x 40 cms. bearings true; date of orig. survey
1907 and that of the subd. survey, June 12, 1998 and was approved on July 16, 1999.”

4. That pursuant to Section 1 Rule 74 of the Rules of Court of the Philippines, and
herein affiant being the only compulsory heir of the deceased ERLINDA L. ALVAREZ, I
hereby adjudicate unto myself the abovementioned parcel of land and the residential house as the
sole owner thereof.
