Case Description

Mr. Lee, a 57-year-old Chinese male, presented to the accident and emergency for left-sided
weakness and sudden difficulty speaking with drooling from the left side of his mouth. His
wife had called an ambulance after his condition worsened . His wife noted that he has had
generalized body weakness and a slightly unsteady gait for a month but he denies any
problems or difficulties. Mr. Lee has a history of uncontrolled hypertension and
hyperlipidemia, benign prostatic hyperplasia (BPH) and Myocardial Infarction (MI) a year ago.
He has a drug allergy to penicillin. His symptoms progressed over the next ten minutes until
he could not lift his arm and has trouble standing. Mr. Lee continues to deny any problems.
His wife sat him on a chair and called the ambulance.

Session 1: Mr. Lee has asked you to assist him to move from his bed to the commode, as he
would like to go over to the toilet to empty his bowel.

Session 2: Mr. Lee has asked for you to assist him to move upward on his bed and turn him to
side lying position.

Session 3: At 3pm, Mr. Lee feels weak and lethargic. The doctor has requested for Mr. Lee to
be send over to the diagnostic department for some investigation The staff nurse has asked
you to assist Mr. Lee to move from his bed to the trolley (using a pat slide).

Session 4: At 7pm, you decided to use a hoist to assist Mr. Lee onto the armchair at his
mealtime.

Duration Plan
20 minutes Tutor Demonstration
 Minimal Assistance Transfer by 1 person
 Moderate Assistance Transfer by 1 person
 Maximal Assistance Transfer (requires 2 people)

30 minutes Clinical Teaching (Using APIE)


- Demonstration: transfer from Bed to Wheelchair/Commode
- Refer to case scenario and discuss the required nursing care for Mr Lee
- Refer to the Braden Scale and determine the level of assistance required by
Mr Lee

30 minutes Practice Session

10 minutes Session 2: Moving a patient up in bed, turning and positioning


 Moving a patient upward onto her bed and turn her to side lying position

10 minutes Session 3: Assisting a patient from bed to trolley using pat slide
 Assisting a patient to move from bed to trolley (using pat-slide)
10 minutes Session 4: Transferring a patient using a powered full body sling lift (hoist)
 Assisting a patient from bed to armchair (using hoist)

The above agenda is a suggested schedule. Your lab tutor may adjust the schedule according to
the needs of your lab group.
