SWORN STATEMENT

I, JONATHAN PENASO, legal age, married, residing at BRGY. CONSUELO, MAGSAYSAY, after having
duly sworn in accordance with the law hereby dispose and state THAT;

That I saw Mr. Vera went to Wellness Center of Quickie Motel Located at the ground floor and I was
hired as personal masseur by Mr. Bartolome S. Vera on the evening of July 17, 2022;

That we agreed on the standard massage service rate of five hundred pesos( 500.00) good for 2
hours whole body massage;

That we checked in upstairs at Quickie Motel and Spa around 8:30 PM and I gave Mr. Vera a fully
body massage as requested;

When I was done with the body with the body massage, he asked me to stay for a while. He wanted
extra service. He gives me the payment for the massage already;

That I was not sure what he meant by extra service, because he started touching my arms and
grabbed my crotch. I hesitated and said “NO” but he insisted and give another five hundred pesos bill.
Eventually, I said “YES”;

That I went to the bathroom to get undressed and took a shower and when I got out, I saw Mr. Vera
Sniffing a white powder like substance on the bedside table;

I was caught by surprise because of what I saw. So I immediately exited the room. I noticed him
running after me, due to the fear I felt when I saw them running after me. I sprinted and went by the
back stairs of Quickie;
 I hid in the basement and waited for a long time and when it was already 12 midnight when I felt
that the area is clear, I exited the Motel;

That I executed this affidavit to attest the truthfulness of the above narrated facts. I hereby affix my
signature this 19th day of JULY, 2022 at GINGOOG CITY.
