Agreement

Whereas being a Scholarship Recipient is a privilege and not a right, the ISNA Scholarship
Committee requires that the agreement as stated hereunder be followed strictly without
exception. Failure to abide by said requirements may result in the loss of part or all of the
scholarship.

Ethical and Moral Standards

Scholarship recipient is expected to exercise the highest moral and ethical behavior, and to act as
an ambassador of ISNA.

Grade Point Average

Recipients must maintain a minimum cumulative grade point average (GPA) of 3.00 for all
credits earned during the period of time under scholarship award. Within two weeks of the
conclusion of any given semester, the recipient is to submit a copy of his or her grades to the
ISNA Student Programs Coordinator. Failure to maintain the required GPA may result in the
scholarship being withdrawn, and monies received to be repaid to ISNA.

The Scholarship will cover the following:

• A one time payment toward tuition and required fees, paid directly to the recipient’s
college or university. No checks will be issued directly to the student.

I have read the terms of the Scholarship Agreement and hereby agree to its terms and
requirements.
• I agree that violation of any part of this agreement may lead to the forfeiture of all
funds awarded. I also agree that in cases of extreme neglect or fraud the ISNA
Scholarship Committee reserves the right to demand full reimbursement of all
funds paid to, or on behalf of the recipient.
• I understand that if this form is not returned to the Scholarship Manager before the
deadline provided that I have forfeited the funds and they will become available for
another applicant.
• I understand that if my tuition is paid in full by other scholarships that I forfeit this
scholarship and the funds become available for another applicant.
