Heads All Other Teaching and Non-Teaching Personnel This Schools Division Pursuant to DepEd Memorandum No, 22, s. 2015 entited Annual Physical Examination of DepEd Employees (Teaching and Non-Teaching Personnel), as a manifestation of concer by this office on the health status ofits employees, all teaching and non-teaching are required to undergo Annual Physical Examination (APE). This is to ‘monitor the health status and physical well-being of employees and to ensure early detection and prevention of ailments, Based on the result ofthe APE in the previous years, there isa significant increase in the incidence of urinary tract infections, hypertension, Diabetes Mellitus, and cardiovascular diseases which affect both young and older ‘employees. To continue monitoring the progress of treatment and the success of lifestyle modification, the following diagnostic and screening procedures are recommended: ‘a. Chest X-Ray (except for pregnant women). Pregnant women are required to submit Sputum Analysis in lieu of Chest X-Ray. An Oral Examination shall also be conducted by the Division Dentist and must be done following the protocols stated under DOH Memorandum No. 2020-0327 or the Interim Guidelines on the Management of Patients and Delivery of Oral Health Services during the COVID-19 Pandemic. 6. Strict compliance to the [ATF Protocols and minimum public health standards must be observed while complying the medical examination in the premises of health facilites. 7. Laboratory results shall be attached to the CS Form 86, duly signed by a Physician for medical clearance and shall be submitted to the District Office through the assigned Schoo! Health Personnel for the field personnel, and to Nurse Grace Concepcion B. Nicasio or Nurse Kirsten Aileen D. Sabanal for SDO personnel on or before June 30, 2022 8. For SDO employees, results attached to the CS Form 86 duly signed by the Physician shall be submitted to for consoldation 9. School/District Nurses shall prepare a Master List of Personnel who have undergone APE. Consolidated reports shall be submitted to this office on or before July 8, 2022. 10.
