1. Pregnancy conceived through artificial insemination and/or assisted conceptions including any condition that arises directly or
indirectly due any complications resulting from the fertility treatment, other than pregnancy conceived via In-Vitro Fertilization (IVF),
Intrauterine Insemination (IUI) and Intracervical Insemination (ICI);
2. Pregnancy with more than two foetuses;
3. Any Pre-existing Illness prior to the effective date of the policy;
4. Hospitalisation primarily for investigatory purposes, diagnosis, X-ray examination, general physical or medical examinations, not
incidental to treatment or diagnosis of a covered Disability or any treatment which is not Medically Necessary and any preventive
treatments, preventive medicines or examinations carried out by a Physician, vitamins/food supplements and treatments specifically
for weight reduction or gain;
5. Any treatment or investigation which is not Medically Necessary, or convalescence, custodial or rest care;
6. Human Immunodeficiency Virus (HIV) and/or any HIV related illness including Acquired Immune Deficiency Syndrome (AIDS) and/or
any mutations, derivations or variations thereof;
7. A self-inflicted injury or attempted suicide while sane or insane;
8. Intoxication by substance, alcohol or drugs not prescribed by a medical practitioner;
9. Dental care and/or treatment, cosmetic surgery, eye glasses, corrective aids and treatment of refractive errors or any optional surgery;
10. Experimental or unlicensed treatment or test;
11. Any event giving rise to a claim on the Insured caused directly or indirectly by the intentional act of the policy owner or person who will

• Restricted interest and repetitive behaviors – inflexibility of


behavior, difficulty coping with change, or other
restricted/repetitive behaviors appear frequently enough to be
obvious to the casual observer and interfere with functioning in a
variety of contexts. Distress and/or difficulty changing focus or
action.

2. Attention Deficit Hyperactivity Disorder - Severe
The presence of symptoms in excess of those required to make the diagnosis of
Attention Deficit Hyperactivity Disorder (ADHD), or the presence of several ADHD
symptoms that are particularly severe; or the ADHD symptoms have resulted in
marked impairment in social or occupational functioning.

The symptoms of ADHD are as below:


• Combination of all three core features; Inattentive, Hyperactivity and
Impulsive are present and ADHD is diagnosed when more than or equal to (“
≥”) six (6) symptoms of hyperactivity/impulsivity and ≥ six (6) symptoms of
inattention have been observed for ≥ six (6) months.
• Inattentive – ADHD is diagnosed if ≥ six (6) symptoms of inattention (but less
than (“<”) six (6) symptoms of hyperactivity/impulsivity) have persisted for ≥
six (6) months.
• Hyperactive/Impulsive – ADHD is diagnosed if ≥ six (6) symptoms of
hyperactivity/impulsivity (but < six (6) symptoms of inattention) have been
present for ≥ six (6) months.

Individual with ADHD may have both; Inattention and Hyperactivity/Impulsivity,


or either one of them may predominate.

• A dysfunction of the left side of the brain (cerebral cortex) and
does not change with age.

• Dyslexia causes a learning disorder that involves difficulty reading


due to problems identifying speech sounds and learning how

4. Gross Motor or Speech Developmental Delay
The inability to perform either of the following tasks by the actual attained age of
twenty-eight (28) months from the date of birth of Insured as confirmed by a neuro-

for support but not aided by a person; OR walk without aid over a distance of two (2)
meters; or

• Say at least three (3) words other than “Papa/Mama”, which mean the same things
each time he/she uses them; OR speak simple words such as “Papa/Mama”; OR use
spontaneous (non-echoed/nonamidated) 2-word phrases.

• The assessment and associated evidence of the inability to carry out the required
tasks must be performed when the Insured has attained the age of twenty-eight (28)
months or older.

• Any identifiable post-natal cause resulting in the inability to perform any of the above
tasks will be excluded.

• Any other independent neurodevelopment assessments done for gross motor and/or
language development may be required in order to adjudicate this Benefit.

Child Development Disorder T&C
If the Insured is diagnosed by a registered psychiatrist in Child Development Disorder, We shall
reimburse the Reasonable and Customary Charges incurred for the Medically Necessary treatment of
Child Development Disorder subject to the following conditions:

• The Insured has undertaken any one (1) of the following therapies:
- cognitive behavioral therapy,
- occupational therapy,
- speech therapy, or
- other relevant therapy which is to be provided by a registered para-medical professional in a
registered care centre on recommendation of the treating psychiatrist;

• The diagnosed condition persists for a period of at least six (6) months despite optimal therapy.

• The Insured is below the age of five (5) years.

The aggregate total amount of benefits payable under Hospitalization Benefit due to Covered
Congenital Conditions and Child Development Disorder Benefit shall not exceed the annual limit as
stated in the Schedule of Benefits, where applicable.

1. Disseminated Intravascular Coagulation

❖ Over-activation of the coagulation and fibrinolytic


system resulting in micro vascular thrombosis,
consumption of platelets and coagulation factors, and
major haemorrhage requiring treatment with frozen
plasma and platelet concentrates.

❖ Only Disseminated Intravascular Coagulation of the


mother caused as a result of complication of
pregnancy is covered.

❖ The diagnosis of Disseminated Intravascular

Exclusions
1. Any Pre-Existing Condition;
2. In Our opinion, the Insured is diagnosed to have disease(s) that is caused directly or indirectly by the existence of Acquired Immune
Deficiency Syndrome (AIDS) or by the presence of any HIV infection. We reserve the right to require the Insured to undergo a blood test for
HIV as a condition precedent to acceptance of any claim. The exception is when HIV Infection is Due To Blood Transfusion. For the purpose
of this Policy:
(a) The definition of AIDS shall be that used by the World Health Organization in 1987, or any subsequent revision by the World Health
Organization of that definition; and
(b) Infection shall be deemed to have occurred where blood or other relevant test(s) indicate in Our opinion either the presence of any
HIV or Antibodies to such a Virus.
3. Self-inflicted injuries while sane or insane;
4. Signs or symptoms of any of the covered conditions/benefits (including those leading to performance of the surgery if required) which first
occurred within the Waiting Period. In the event of any conflict or discrepancy of opinions relating to the signs or symptoms of covered
conditions/benefits and their manifestation between a Physician and the Insured, We will adopt and follow Physician’s professional
opinion;
5. Willful misuse of drugs or alcohol;
6. Elective termination of pregnancy other than for medical reasons under Female Pregnancy Benefit.
7. We shall not pay for any injuries due to Accident or Skin Grafting Due to Burns claim arising directly/indirectly from any one of the
following occurrences:
(a) Resulting from suicide, self-inflicted bodily injuries while sane or insane, disorderly conduct on the part of the Insured, or upon the
Insured deliberately exposing herself to unnecessary danger;
(b) Being involved in an unlawful act;
(c) Sustained while under the influence of narcotics or alcohol;
(d) Sustained while engaged in hunting, mountaineering or racing in motor vehicles or motor boats or any hazardous activities;
(e) Consequent upon war (declared or undeclared), invasion, acts of foreign enemies, terrorist activities, hostilities or warlike operations,
riot and civil commotion, industrial action or revolution;
(f) Being involved in any travel or flight in or any type of aircraft except on a regular scheduled passenger flight or a commercial aircraft;
or
(g) Being involved in any elective surgery, cosmetic or plastic surgery, other than for conditions specifically covered under the Medical
Complication due to Elective Cosmetic Surgery benefit.

These are unit deducting riders which are plans targeted at specific market needs. These plans are attachable to an Investment Linked Insurance plan..

A-Plus GenNext
❖ A-Plus GenNext is a unit-deducting rider attachable to a regular premium Investment-Linked Insurance plan i.e. A-Life Link 2 and A-Life Joy 2. This plan offers
protection and allowance for young adults to overcome life’s uncertainties in pursuit of their dreams.
FEATURES

❖Provides a monthly Cash Allowance when an accident or illness prevents you from performing your usual work for at least 30
consecutive days.
❖Provides a lump sum benefit so you can focus on recovery when you are struck with a critical illness or accidental disablement.
❖Provides a monthly Cash Allowance to help you through the challenging period when struck with early-stage cancer or early-stage
heart-related illnesses.

❖Indication of Cost of Insurance (per RM 1,000 rider sum assured) for a male of standard health:

▪ Assault or murder ▪ Participation in any fight or affray
▪ Riot and civil commotion, industrial action or terrorist activity ▪ Racing on horse or wheels
▪ Willful exposure to danger or attempted self-destruction or self-inflicted injuries while sane ▪ Accident occurring while or because the Insured is under the influence of
or insane alcohol, any non-prescribed drug or illegal drug
▪ War, declared or undeclared, or revolution ▪ Pregnancy, miscarriage or child birth
▪ Service in the armed forces in time of declared or undeclared war or while under orders for ▪ Mental or nervous disorders, depression due to any reason, learning

• Amount of benefit will be a 1% of the initial Rider sum assured. For Benefit 1, the Amount of Benefit will be a % of the balance sum assured after a claim is made under Benefit 1b or 1c; if any
• Benefits 2, 3 and 4 will only be claimable as long as the Rider is still in force and the amount of benefits of these categories do not reduce the initial Rider
sum assured.
• Once 100% of the initial Rider sum assured has been fully paid out and the Rider terminates due to claim on Benefit 1, then Benefits 2, 3 and 4 will terminate as well, with the exception of Benefit 2a (Breast Lumpectomy), Benefit 2b (Mastectomy) and

Definition Of The Additional 6 CIs
Additional 6 CIs Definition
Poliomyelitis Unequivocal diagnosis of infection with the polio virus leading to paralytic disease as evidenced by impaired motor function or respiratory
weakness and resulting in the total inability of the Life Assured to perform (unaided) at least three (3) of the Activities of Daily Living for a
continuous period of at least three (3) months.
Progressive A systemic collagen-vascular disease causing progressive diffuse fibrosis in the skin, blood vessels and visceral organs. This diagnosis must be
Scleroderma unequivocally supported by biopsy and serological evidence and the disorder must have reached systemic proportions to involve the heart,
lungs or kidneys.
The following are excluded:
a) Localised scleroderma (linear scleroderma or morphea);
b) Eosinophilic fasciitis; and
c) CREST syndrome.
Apallic Universal necrosis of the brain cortex with the brainstem intact. This diagnosis must be definitely confirmed by a consultant neurologist
Syndrome holding such an appointment at an approved hospital. This condition has to be medically documented for at least one month.
Chronic An unequivocal diagnosis of Chronic Relapsing Pancreatitis, made by a specialist in gastroenterologist and confirmed as a continuing
Relapsing inflammatory disease of the pancreas characterised by irreversible morphological change and typically causing pain and/or permanent
Pancreatitis impairment of function.
The condition must be characterised by:
(a) recurrent acute pancreatitis for a period of at least two (2) years;
(b) generalise calcium deposits in pancreas from imaging study; and
(c) chronic continuous pancreatic function impairment resulting in mal-absorption of intestine (high fat in stool) or diabetes.
Relapsing Pancreatitis caused directly or indirectly, wholly or partly, by alcohol is excluded.
Elephantiasis The end-stage lesion of filariasis characterised by severe and Permanent edema of arm and leg or other part of body as a result of lymphatic
obstruction.
Unequivocal diagnosis of elephantiasis must be clinically confirmed by a relevant specialist, including laboratory confirmation of microfilariae.
Lymphedema caused by infection with a sexually transmitted disease, trauma, cancer, post-operative scarring, congestive heart failure,
radiation or congenital lymphatic system abnormalities is excluded.
Creutzfeldt- A neurological disease, fatal spongiform encephalopathy, accompanied by signs and symptoms of:
Jakob Disease (a) uncontrolled muscular spasm, tremor;
(Mad Cow (b) severe progressive dementia;
Disease) (c) cerebellar dysfunction; and
(d) athetosis.
The diagnosis must be made by a specialist in neurology acceptable to us and must be based on conclusive electroencephalography (EEG) and
cerebrospinal fluid (CSF) findings as well as computed tomography (CT) scan and magnetic resonance imaging (MRI).

7 Encephalitis
❖ Low Severity: Encephalitis with full recovery

❖ High Severity: Encephalitis with permanent neurological
deficits

❖ Severe inflammation of brain substance (cerebral hemisphere, brainstem or cerebellum) caused by viral
infection requiring hospitalization. The diagnosis must be confirmed by a consultant neurologist and
supported with appropriate investigations proving acute viral infection of the brain.

❖ Encephalitis in the presence of HIV infection is not covered.

7 High Severity – Encephalitis with permanent


neurological deficits
❖ Severe inflammation of brain substance, resulting in permanent functional impairment.

❖ The permanent functional impairment must result in an inability to perform at least three (3) of the
Activities of Daily Living. A minimum Assessment Period of thirty (30) days applies. The covered event
must be certified by a neurologist.

❖ Encephalitis in the presence of HIV infection is not covered.

8 Low Severity – Bacterial Meningitis with full recovery

❖ Bacterial infection resulting in severe inflammation of the membranes of


the brain or spinal cord which requires hospitalization. This diagnosis
must be confirmed by: (a) The presence of bacterial infection in
cerebrospinal fluid by lumbar puncture; and (b) A consultant neurologist.
❖ Bacterial Meningitis in the presence of HIV infection is excluded.

High Severity – Bacterial Meningitis with permanent neurological deficits

❖ Bacterial meningitis causing inflammation of the membranes of the brain
or spinal cord resulting in permanent functional impairment. The
permanent functional impairment must result in an inability to perform at
least three (3) of the Activities of Daily Living. A minimum Assessment

i. an appropriate specialist; and
