2. That this affidavit is being executed to support my application for the renewal of my
accreditation as material engineer in connection with the issuance of a new Certificate
of Accreditation of Contractors’ and Consultants’ Materials Engineers.

3. That I hereby testify under oath that I am not connected with any government
institution or instrumentalities.
4. That I am executing this affidavit to attest the truth of the foregoing facts.
