Greetings! We are a group of researchers from Quezon City Science High School, and we are

delighted to inform you that through our sampling method, you have been chosen to participate in our

study. Answering this survey is voluntary, and part-taking in our research will not result to negative

consequences of any kind.

The purpose of this study is to assess and compare the knowledge, attitude, and practice of the

Quezon City residents from District 3 and 5.

CONFIDENTIALITY DISCLOSURE AGREEMENT (CD Agreement)

We, the researchers, solely pledge to utilize the information gathered from the

respondents only for the purpose of fulfilling our study “ A Comparative Study of

Knowledge, Attitude, and Practice on Medical Waste Disposal Management among the
