Sub: Request for transfer from OCP III, Rg Area -2 to Mandamarri Area – Reg.

I am a General Mazdoor, working at OCP - 3, Rg Area - 2 and would like to bring the
following for your kind consideration.

With deep concern I wish to convey my problem and the necessity that my son,
SRI VARSHITH, 10 Years old, suffers from the rarest health problem which is
diagnosed as CHILDHOOD ATAXIA, progressive ataxia. Our whole family is
worried about his health. First, he was provided treatment at National Institute of
Mental & Neuro Health at Bangalore. Now, he is under treatment at KIMS,
Hyderabad, regularly, with close observation.

Regular Physiotherapy under the doctors is advised as the main remedy and he is
being provided the therapy at our Ramakrishnapur Area Hospital as I am staying at
my native place, Mandamarri with my parents. These doctors have warned not to be
absent even for one day.

My parents are old and with degenerative weaknesses. I am taking my son for the
therapy, which is most important for recovery of his health, and it is going to be
difficult attending duties at a faraway place of OCP-3, Rg -2.
Under such necessity. I wish to be transferred to the Mandamarri Area, to avoid my
difficulty.

Therefore, I request your honor to kindly accord your sanction and arrange to
transfer me to Mandamarri Area, compassionately, and do needful.
