Province of Masbate, after having been duly sworn to in accordance with law,
do hereby depose and state that:
1. I was married to the deceased Remedios G. Quintal;

2. I have used the name Abundio Quintal in all of my records and

dealings until it was changed to ABUNDIO QUINTAL SR when our
son Abundio Quintal Jr. was born;

3. I have acquired a property located at Milagros, Masbate City which

was tax declared under my name Abundio Quintal;

4. I am executing this affidavit to attest that ABUNDIO QUINTAL SR.

and Abundio Quintal appearing in the Tax Declaration of the
abovementioned property are one and the same person;

5. I am requesting for the correction of name from Abundio Quintal to

Abundio Quintal Sr. in the Tax Declaration:

6. I am executing this affidavit to attest to the truthfulness and honesty

of the foregoing facts and for all intents and purposes it may serve.
