A Spontaneously and consistently identifies ethical and legal issues and addresses them proactively. Judgement is

HI Initiates supervisory discussion regarding areas of inexperience in personal differences when needed.
I Has significant lack of knowledge or discomfort regarding some patient groups, but resolves such issues
effectively through supervision. Open to feedback regarding limits of competence.
E Is beginning to learn to recognize beliefs which may limit effectiveness with patient populations.
R Has difficulty recognizing or working around prejudices, holds some beliefs which limit effectiveness with certain
patients and unable to surmount these problems to date in supervision.
A Proficiently administers all tests. Completes all testing efficiently. Autonomously chooses appropriate tests to
answer referral question.
HI Occasional input needed regarding fine points of test administration. Occasionally needs reassurance that selected
tests are appropriate.
I Needs continued supervision on frequently administered tests. Needs occasional consultation regarding
appropriate tests to administer.
E Test administration is irregular, slow, occasionally awkward.
R Significant difficulty obtaining valid test administration.
A Spontaneously and consistently applies supervision skills. Supervisee verbalizes appreciation of trainee’s input.
HI Consistently recognizes relevant issues, needs occasional guidance and supervisory input. Well thought of by
supervisee. Supervisee recognizes at least one significant strength of trainee as a supervisor as documented on
evaluation form.
I Generally recognizes relevant issues, needs guidance regarding supervision skills. Supervisee finds input helpful.
Trainee is rated by supervisee at the satisfactory or higher level in all areas.
R Unable to provide helpful supervision.
SUPERVISORS ARE REQUIRED PREPARE A ONE PAGE FORMAL SUMMARY OF THE STUDENT’S WORK DURING THE
PRACTICUM. THIS SHOULD INCLUDE THE NUMBER AND TYPE OF CASES THE STUDENT HAS SEEN, SPECIFIC ETHNIC ORIGINS
OF CLIENTS, THERAPEUTIC APPROACHES USED, AND THE OUTCOME OF THERAPY OR ASSESSMENT.
GUIDELINES
In the rare situation when it is recognized that a trainee needs remedial work (Rating of R), a competency
assessment form should be filled out immediately, prior to any deadline date for evaluation, and shared with the
trainee and the Director of Training. In order to allow the trainee to gain competency and meet passing criteria
for the rotation, these areas must be addressed proactively and a remedial plan needs to be devised and
implemented promptly.
All applicable competency areas will be rated at a level E or higher by the end of the practicum. No applicable
competency areas will be rated R.

SPECIALTY AREAS TAKEN FOR BREADTH AS OPPOSED TO PART OF THE RESIDENT ’S MAJOR AREA OF
TRAINING & COMPETENCY)
Note: In many minor rotations or intensive psychotherapy experiences, the resident will be expected to
progress to a rating of I or higher by the end of the training period. In some specialty area rotations that are
being taken for breadth (as opposed to part of the resident’s major area of training), however, it may be
acceptable (if agreed upon by the major supervisor and Training Director) for the resident to obtain a rating of E
at the end of internship (e.g. a neuropsychology rotation for a non-neuropsych stream trainee, child
psychotherapy for an adult stream resident or vice versa, etc).
All competency areas will be rated at a level of competence of HI. Only areas where the post-doc has
no prior experience, and will not be a part of the individual’s major area of competence for practice,
will be rated I. No applicable competency areas will be rated as R or E.
Supervisors and supervisees are expected to discuss and come to an agreement on core competencies
(principal areas of competence for licensure/autonomous practice) that will be evaluated over the
course of the post-doctoral supervision year. In all such areas of core competency, supervisees should
achieve a rating of A by the end of the post-doctoral residency period. Some less core areas may
receive a rating of HI. No applicable competency areas will be rated as R or E. Some specialty area
experiences that would take a more intensive course of study to achieve a higher level of competency,
but will not be a part of the supervisee’s area of declared competence for autonomous practice, may be
rated I.

The trainee HAS successfully completed the above goal. We have reviewed this evaluation
together.
The trainee HAS NOT successfully completed the above goal. We have made a joint
written remedial plan as attached, with specific dates indicated for completion. Once completed, the
rotation will be re-evaluated using another evaluation form, or on this form, clearly marked with a
different color ink. We have reviewed this evaluation together.
