AFFIDAVIT OF DISCREPANCY IN NAME

I, ZENITHSYL P. ESCUTON, of legal age, Filipino and a resident of El
Progreso Buruanga, Aklan, after having been duly sworn to in accordance with
law, do hereby depose and state:
1. That I am an active member of Social Security System with the
abovementioned registered name;

2. That I gave birth through normal delivery on April 9, 2022;

3. That in the birth certificate of my child, Prince Joel P. Sanchez, the name
that was entered was ZENITHSYL MONSERATE PRADO, which is my
maiden name;

4. That I, ZENITHSYL P. ESCUTON and the one ZENITHSYL MONSERATE

PRADO, being entered in the abovementioned birth certificate are one
and the same person, the herein affiant;

5. That I am executing this Affidavit to attest to the foregoing facts and for
any legal purpose and intent it may serve;
