Staff To work as part of an inter-professional team with supervision
from a senior physiotherapist. Independently managing a
caseload of patients who require comprehensive assessment
and rehabilitation within the acute sites and community
settings. To educate other professionals, other disciplines,
therapy assistants and students within the inter-professional
team on physiotherapy.
Information To ensure accurate, comprehensive and up-to-date clinical
Systems records are maintained in accordance with the Service Trust
guidance and professionally agreed criteria.

Job purpose To provide specialist physiotherapy to patients within an acute


setting as part of an inter-professional team. Participating as a
member of the team; managing a clinical caseload within the
protocols, objectives and procedures of the team.
To support service improvement and delivery within the team.
Participate in health promotion and education of staff, clients
and families.
The post holder will undertake regular continual professional
development activities.
Other To support discharge planning across acute sites within East
Sussex Healthcare Trust.

Colleagues & Daily / as The post holder will regularly communicate


Outside required with and provide advice to patients, relatives,

concerning the patients’ clinical presentation,
home situation and utilise holistic approach to
support admission avoidance/discharge
planning that optimise patient centred
approach where possible. They are also
required to negotiate goals with patients and
motivate them to participate in treatment/
discharge plans to achieve these goals. The
post holder may be required to convey difficult
or sensitive information or deal with
challenging or aggressive clients/relatives with
support as required.

Key duties and responsibilities

Clinical

- To undertake all aspects of clinical duties as an autonomous Physiotherapy
practitioner with developed cross discipline competencies.

- To be professionally and legally accountable for a designated patient caseload


and to decide priorities for own work area, balancing other patient related and
professional demands.

- To undertake initial assessment of patients using the single assessment


process.

- To undertake the comprehensive assessment of patients including those with


diverse or complex presentations. To use a range of standardised assessments
and functional assessment techniques. To use advanced clinical reasoning
skills to analyse and select appropriate course of treatment.

- To work with critically ill patients which results in unpredictable situations &
managing risks at the patient place of residence.

- To work across multidisciplinary team including nurses, OTs, PTs, ISSWs &
ISWs.

- To delegate appropriately and provide supervision for Band 5 team members


and rehabilitation support workers.

- To support clinical placements of Physiotherapy students within your rotation;


where applicable take the lead in developing their programme of study within
the placement, clinical supervision and formal assessment.

- To undertake assessment of clinical risk as part of everyday practice and to


record and report as appropriate seeking guidance from senior clinician where
appropriate.

- To liaise closely with the medical, nursing, AHP staff, social services and acute
or rehabilitation staff to promote greater understanding of admission avoidance
approaches and discharge planning options including rehabilitation.

- To formulate and deliver an individual treatment programme based on a sound


knowledge of evidence based practice and treatment options using clinical
assessment, reasoning skills and knowledge of treatment skills e.g. facilitation
of movement, tonal management, gait sensory and balance re-education, client
and carer education, health promotion, provision of equipment, functional
programmes, activity analysis, and grading of activity programmes, through a
patient centred intervention.

- To evaluate patient progress, reassess and alter treatment programmes/out of


hospital management or discharge plans if required.

- To assess patients in their own home to identify need of care to prevent


hospital admission.

- To ensure professional registration is maintained with HCPC and work within a


professional code of ethics e.g. CSP, HCPC.

- To participate in the development of an inter-professional working environment

Managerial

- To contribute to the team / service development- (Identifying opportunities to


improve service delivery in order to provide best patient care in relation to
resources available and evidence based practice) with the assistance of Team
Leader.

- To undertake specific tasks as designated by senior staff/ team leader.

- To participate in team meetings to ensure effective exchange of information


within the interdisciplinary team for best patient care and co-ordination of local
service delivery.

- To be involved in the student / RSW and Band 5 training programs.

- To build relationships between acute and community care to promote seamless


care.

- To maintain accurate record keeping and client data information whether written,
electronic or verbal in line with the Service, Trust and CSP guidelines.

- To participate in clinical audit and service developments. (To be done through


regular measurement/ data collection, work evaluation using evidence based
practice projects, audit and outcome measures, either individually or with other
clinicians.)

- To actively deputise for the Team Lead in their absence including rotas,
allocation of tasks, staff development and performance, incidents and complaints.

Education

- To be responsible for maintaining own competency to practice, through


continuing professional development activities including reflective practice, review
of relevant literature, participation in local in-service training programs and peer
review, maintenance of a personal portfolio and the attendance of relevant training
courses as identified within personal development plan.

- To facilitate and contribute to clinical development within the team to ensure that
current practices and practice developments are evidence based and up to date.

- To provide and implement education formally and informally to health and social

care professionals and the public, with support from senior staff as required.

- To share skills within the inter-professional team to promote a greater


understanding of the profession and improve patient care as a result.

- To be an active member and contribute to the delivery of the in-service training


programme with support of senior staff, undertaking student training /education of
students of all disciplines that work within the team and rehabilitation support
workers.

Statement
