Dear Dr. Acidera,

Blessed Day Sir! I am writing this letter to request permission to be included on the
list of students allowed for deployment in cooperating school for field study.

The purpose of undergoing field study for 5 weeks is very essential part as pre-service
teacher. It prepares us to the next level of our journey, to become effective and
efficient future teacher. All the learning that I will help me to achieve my goals. With
that, I am seeking your approval to be part of field study students.

I am in progress (IP) in research education, TTL2 and calculus because of delayed

inputting of grades in SIAS. Due to my fault because of late submission of activities.
Another IP subjects is the ALS2 and ABSTRACT ALGEBRA due to unfinished
activities. I already approach my instructor regarding this IP’s I have.

With these thoughts, your permission to allowing me to be deployed even having an

IP is a big factor in pursuing my dreams to because an educator. Have a blessed day to
