To provide a mechanism to facilitate the inpatient admission of patients to Springleaf Health Care

SCOPE:

 Springleaf Health Care, Bangalore welcomes all patients. No patient shall be denied Admission due
to race, color, religion, ancestry, financial class or national origin.

 Patients may be admitted to Springleaf Health Care, Bangalore by a Consultant. A list of such
consultants shall be available with the IP Billing Counter through the HIS. The Consultants/Senior
Consultants shall fill up the “Request for Admission form”.

 Acceptance of non-emergency admissions and transfers to Springleaf Health Care, Bangalore shall
be made contingent upon verification of available resources through IP Billing Counter/ Emergency.

TYPES OF ADMISSIONS:

 Planned admission- A planned admission is an inpatient admission of a patient pre-planned in


advance of the patient’s presentation for inpatient care. All required admission paperwork (including
the Authorization letter of the TPA) shall be submitted prior to the date upon which the patient is to
be admitted. The admission case sheet shall be printed and the patient shall be escorted to the
appropriate inpatient bed.

 Admission from Out Patient - Patients may be directly admitted by Consultants from their offices
in the outpatient clinic. The request for admission form shall be completed in the clinic and
submitted to the Admitting Desk. The IP Billing Counter shall complete necessary admission
processes and the patient shall be escorted to the assigned bed. Authorization from insurance
/corporate are proceeded in parallel.

 Admissions from the emergency room- Emergency room patients requiring inpatient admission
shall have the request for admission form and other required information forwarded to the Admitting
Desk. The IP Billing Counter shall complete the admission process and the patient shall be
transported from the emergency room to the assigned bed. Authorization from insurance /corporate
are proceeded in parallel.

UNIQUE IDENTIFICATION NUMBER:


 Patients not registered earlier shall be registered and a UHID shall be generated.

 The IP staffshall generate ID Band on the patients visit for admission.

 This number is used for identification of the patient across the hospital and to ensure continuity of
care for all services pertaining to the Hospital.

 The organization has identified the following hospital, to which the staff will direct the patients in
case of non-availability of beds.
1. Narayana Hrudalaya, Bommasandra.
2. As per patient wish.

TRANSPORT & REFFERALS:

When resources matching the patient needs are not available at (SHPL) Springleaf Health Care,
Bangalore, Appropriate transportation arrangements and a medical escort (if needed) shall be made
through the Chief Medical Officer/ Physician.

Following steps are done to provide safe and appropriate care during transit.

 Transportation provided or arranged is appropriate to the patient needs in terms of appropriate


equipment, supplies and medications. The needs are assessed by the Chief Medical Officer/
Physicians/consultant

 Patients shall be transferred to facilities that can meet the patient’s needs. Appropriate transportation
arrangements and a medical escort (if needed) shall be made through the Chief Medical Officer/
Physician/consultant

 The transport vehicles provided bySpringleaf Health Care, Bangalore shall meet the required laws &
regulations.

 The ambulance driver has the responsibility of checking the condition of ambulances like
functioning status of the lights, siren, beacon lights, etc. He shall also responsible to make a schedule
for ambulance servicing.

 The transport vehicles shall be kept in good condition with periodical maintenance.

 The transport vehicles shall be monitored for quality and safety including infection control and
complaint management by HOD of Emergency services.
