Join us for our 1st ANNUAL Fun Run, Zumba, Adopt A School For A Day and Planting Activities on August 5, 2022
at 5:00 am. The registration fee is PhP250.00 per runner which shall come with a finisher certificate and an event dri-fit
singlet/shirt. We are also open for sponsorships. Each municipality shall determine their venue for the Fun Run, Zumba, the
school to be adopted for a day and for planting activities. The Adopt a School For A Day is in support to the Adopt A School
Program while the planting shall be done in support to the National Greening Program, Gulayan Sa Paaralan, School in a Garden
and Million Flowers Bloom. The fun run is open all 18 years old and above of all abilities. It means they can participate in the
event whether they run, or whether they walk or whether they wheel to the finish line. It does not matter. We just want you to
come, have fun and support the activity. The Zumba, Adopt A School For A Day and Planting Activities shall be opened to
SPG/SSG officers, as long as they provide a duly signed parent’s consent.

ASSUMPTION OF RISK, RELEASE AND WAIVER OF

LIABILITY AND INDEMINITY AGREEMENT
In consideration of my participation to the SYNCHRONIZED FUN RUN, ZUMBA, ADOPT A SCHOOL FOR A DAY AND PLANTING ACTIVITIES on
August 5, 2022, I agree:

1. That I understand the nature of the activities and my capabilities and physical condition and I believe I am capable and in sufficient physical condition to
participate. I further acknowledge that I am aware the activity will be conducted in facilities open to the public. I further agree and warrant that if at any time I
believe conditions to be unsafe, I will immediately discontinue further participation in the activity but the amount I paid may no longer be refunded. I understand
that I am responsible for consulting a physician before beginning an exercise program and warming up before the Activity.
2.That I FULLY UNDERSTAND that these are physical activities and involve risks and dangers and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS
AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES incurred as a result of my participation in the Activity.
3. That I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY
AND SAVE AND HOLD HARMLESS DEPED, their respective administrators, management, teachers, and DepEd employees.
4. That I am 18 years old and above and I have read and fully understand the nature of the activity.
5. Having been so appraised, the undersigned elects to and does assume all risk for claims, known or unknown, heretofore or hereafter arising from the subject
of this release, and specifically waives any right to sue or ask for indemnity.

I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND IT’S TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY
SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A
COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOW ED BY THE LAW AND AGREE THAT
IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THAT THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL
FORCE AND EFFECT.
