 Improve patient to care giver experience where need be and possible
 Find new and more productive ways to address affordability within the system and all
branches of care
 Find new and improved ways to support each other/team members
 Expand our network clinically and work to improve those relationships with other health
care organizations within the state and eventually expanded outside the state

Questions:
 Did your provider/clinician contact you with conformation of their arrival as well as
arrive on time themselves?
 Did your provider/clinician address all your concerns thoroughly?
 Did your provider/clinician go over all previous medications as well as any other
outstanding information with you to ensure they are caught up?
 Did your provider/clinician explain everything to the point of patient understanding?
 Did you experience any unmanaged pain while on our services that your
provider/clinician did not tend to after being notified?
