CHECKLIST QUANTITY FOR PRC AGENCY TO AGENCY TRANSACTION (DOGH & PRC)

Original PRC ID/License.

a) Submit to DOGH for processing of Authenticated
PRC ID.
Clear Photocopy of PRC ID/License.

1 a) Photocopy of ID must not be resized since PRC

checks if it is the same size with the actual ID.

Board Rating

a) No need to provide original as long as
photocopy of Board Rating is CLEAR.
ORA OHRA Form

a) Strictly must be printed in LONG BOND PAPER

b) 2 copies (For the 1 copy Personal Details will be
blank just sign the Signature Over Printed Name
of Applicant)
PRC Online Schedule ID/License (PRC Davao only)

a) Applicant must schedule with PRC Davao for

processing of authenticated PRC ID.

b) Applicant must pay the processing fee
c) After online appointment has been scheduled
and paid, print the online appointment schedule
and submit to DOH with the other requirements.
1 Documentary Stamp (purple colored)

This requirement (PRC ID Authentication) is not included with the initial request for 3 authenticated copies
of PRC ID.
Do not fasten the documents.
Size of the photocopied document must be exact size of the original document.
