PARENTAL WAIVER AND RELEASE OF LIABILITY FORM

This is to certify that I, parent of _________________, of ______________, allow my son/daughter to

attend the ___________ organized by the _______________, hereby referred to as the “event”.

I understand the details of the event as follows:

Furthermore, I also understand that:

1. This activity is organized solely by students and is not sanctioned by the schools.
2. For the safety and security of the participants, it is my responsibility to arrange drop off to and
precise fetching time of my son/daughter from the ___________. 
3. The organizers of this event are not liable for any of the participants’ actions.
4. Any damage to or loss of property shall be handled by the perpetuator.
5. An antigen test must be done on the day of the event, pictured with a watch or gadget showing
date and time, labeled with the student’s name, and uploaded in the google drive provided. No
negative antigen test result, no entry.
6. Adult chaperone/s will be stationed outside the clubhouse to ensure that no alcoholic beverage
will be served, discourage smoking in any form, and to notify the parents in the event of medical
emergencies.

In view of the above, I hereby agree to be responsible for the conduct and actions of my son/daughter
and to release the event student organizers, student class representatives, parent representatives,
event chaperones, etc from any liability, claims and demands arising from incidents or accidents that
may occur during the event.
