As discussed, we are planning the virtual onsite assessment of the hospital - (Application
no.) – (Hospital name, date of the assessment and Timing )
 
You are requested to kindly go through and fill the attached Consent form and submit the duly signed
document to the undersigned so that we may proceed further.
 
Kindly go through the Guidelines document detailing the assessment process and forward us the
required documents. 
 
The Scope of service excel document is also attached herewith which needs to be submitted along with
the required mandatory photographs (Technical mandatory document sheet as attached). Please save
scope format & open with MS Excel sheet only and send Scope of Services in Excel Sheet. Please fill all
the columns, select at least one option from drop down box for OP, IP & ER, Type of Services. Scope of
services has to be filled as per the specialty in the hospital. Kindly do not change any column and fill it
completely.

Please Find attached the Bank Transfer details of NABH for the payment of the overhead charges as
required.
