1. All learners in AP/HUMSS 1C classes (Grade 1-12) will participate in the Classroom Histo-Quiz to be conducted by every AP/HUMSS 1C teachers in their actual classes.
2. The activity must be reflected in their DLL/DLP for the day.
3. This is one way of popularizing the history of the barangay, municipality, school and the division.
4. Winners for Grade 6 and Grade 7 will participate in the School Histo-Quiz.
October 5. Questions will focus on the history of the barangay / municipality (5 items) history of the school (5 items) and history of the division (10 items).

7. Points for every correct answer shall be given as follows;

8. Sources for the history of the barangay / municipality shall be sourced from the municipal tourism office, the school history is from the School Report Card (SRC) or any
document produced by the office of the school head. Source of the history of the division is the 110th Founding Anniversary Souvenir Program issued by the office to all schools in
2012.

1. All schools are required to prepare necessary documents for their School Histo-Exhibit in commemoration with the celebration of the 120th Founding Anniversary of the
Division.
October 2. The school will exhibit the updated histories of the school, barangay and municipality, records of school establishment, lot title and status of ownership, pictures of the first

in an available room.

1. Champion in the Classroom Histo-Quiz for Grade 6 and Grade 7 will participate in the School Histo-Quiz. This will be handled by the School AP Leader and or AP Department
Head with the participation of the AP teachers under the supervision of the School Heads.

1. Champion in the School Histo-Quiz will participate in the District Histo-Quiz in its’ simplest form to be decided by the District Supervisors and to be implemented by the District
Learning Specialists.
2. Champion in the District Histo-Quiz will compete in the Division Histo-Quiz on October 14, 2022.
3. Questions will focus on the history of the municipality (5 items) history of the school (5 items) and division history (10 items). There shall be a total

Rules and Mechanics


1. Contestants shall bring their own writing sheets (20 pieces 1/2 lengthwise of a long bond paper) during the contest. The monitor shall collect the answer sheets arranged
accordingly. Said answer sheets shall be submitted to the board of judges for their decision.
2. English and Filipino shall be used as the official languages in the conduct of the quiz.
3. There shall be a total of twenty (20) questions to be given, 6 of which are “easy questions”, 7 are “average”, and 7 are “difficult”.
4. Points for every correct answer shall be given as follows;
- one (1) point - for an “easy” question
- two (2) points - for an “average question
- three (3) points -for a “difficult” question
5. Awarding of the winners (First, Second, third, Fourth and Fifth) shall be on the Kick - Off and Awarding Ceremonies of the 120th Founding Anniversary of DepEd Tarlac Province.
Winners will be accompanied by their coach, school head of winning contestant, district supervisor (for elementary), while in the secondary, the winners will be accompanied by
the winning coach, school leader/department head, school head and district supervisor.

Note: Ms Josephine R. Malonzo, President of DASSLE in the elementary, Dr. Milagros N. Castaήeda, Ms. Diana V. Facun andMs. Kathlyn Melegrito will take charge of the venue
and program.

Mr. Harold G. Guiam, President of DASSLE in the secondary, Dr. Paul Gamboa and Mr. Luisito Tejada will take charge of the venue and program.

MUNICIPAL EUCHARISTIC CELEBRATIONS All Parishes 7:00 A.M. PSDSs


In the Municipalities
Participants in the Simultaneous Municipal Eucharistic Celebrations are the following;
Awardees of Division Activities, Public and Private Schools’ SPG and SSG Officials and their Advisers, District Faculty Club Officials, Secondary School Faculty Club Officers, District
October Learning Area Specialists, Secondary School Learning Area Leaders, Department Heads, School Heads (TICs, OICs), PSDSs, Municipal PTA Officials, SGC Officials, Alumni, Municipal
24, 2022 Officials, and others who wish to attend the mass in coordination with the PSDSs of the municipalities based on the seating capacity of the parish church.

Note: Mr. Augusto L Ballesteros will serve as overall chairperson of the simultaneous Eucharistic Celebration. Proper coordination among PSDSs must be done to assure the
success of the Holy Masses and receive blessings and indulgences.

October CLUSTER MOTORCADE Parishes to Maria 8:30 A.M. School Head of the Biggest and Oldest
24, 2022 Cristina Galang Park Secondary School in the municipality
The EPSvr - SGOD as the Division Chairperson, District Supervisors, Secondary School Cluster Chairs, and the School Heads of the biggest and oldest secondary school in the
municipality as Co-Chairpersons with their following roles;

1. assist cluster chairs, secondary school heads in preparing and securing permits to assemble and travel in their municipalities (in front of their parishes) and in converging to
Maria Cristina Park considering their entry points in Tarlac City to manage the traffic situations in their route in going to the venue;
2. Determine the delegates in the cluster and coordinate to the committees.
3. Assign area per cluster in the venue with chairs and advise participants to bring umbrellas.
4. Secure permit for the venue, authority to use/pass the routes of all delegates entering Tarlac City highways, streets, and boulevards to Maria Cristina Park.
5. Assure security, peace and order of participants in their entry routes and venue. Police assistance and visibility.
6. Assign medical team in all activities of the 120 th founding anniversary celebration as reflected in the matrices.
7. Document the motorcade of all clusters using drones.

MOTORCADE ROUTE FROM THE PARISHES TO THE VENUE


Cluster I converge at Bacuit, Tibag, Tralac City (before Cindy’s and proceed to the venue via San Vicente and CLDH
Cluster II converge infront of the WalterMart Paniqui and proceed to the venue via Mc Arthur Hiway, Aquino and Romulo Boulevards
Cluster III Victoria, Gerona & Pura converge at Shell Gas Station Salapungan, Tarlac City and proceed to venue via F Tanedo Street;
JVYap HS will converge with cluster members at Osias Colleges, and;
San Jose Districts will assemble at St. Gerards Academy and wait for the tail of Cluster I delegation but will join Cluster III
delegation in the venue;
Cluster IV converge infront of SM Tarlac City and proceed to venue via Macabulos Drive;
Private Schools will join in their clusters during the motorcade but will stay in their assigned place in the venue
Division Office and converge infront of the Cathedral and proceed to venue via City Hall, Tarlac West, and Macabulos Drive;
TNHS TNHS will join the Cluster III delegation in the venue.

Division Office Personnel will adopt Moriones High School as the venue for the Division Tree Planting, Tree Growing and Tree Caring.

SEARCH FOR THE OLDEST LIVING RETIRED White Hall 1:00 P.M. Dr. Conrado C. Domingo
DEPED TARLAC PROVINCE PERSONNEL
1. The District Supervisors will manage the search on the retired living superintendent, assistant superintendent, division supervisor, district supervisor, elementary and secondary
school principal, elementary and secondary school teacher, and school personnel in their districts.
2. The retired living personnel must submit any two of the following documents (birth certificate, baptismal certificate, senior citizen identification card, GSIS UMID Card) to be
validated by the district supervisor and team.
October 3. Only the document of the oldest living personnel shall be properly submitted to Dr. Conrado C. Domingo on or before October 20, 2022 ready for division search on October
25, 2022 21, 24,and 25, 2022.
4. Winners on the search will be declared on October 25 by the Chairperson duly signed by the TWG.
5. An invitation letter will be sent by the Committee on Invitation under Dr. Helen R. Bose through the PSDSs on this date.
6. Awarding of the winners will be on October 27, 2022 during the Thanksgiving and Awarding Ceremonies at the Bulwagan ng Kanlahi
7. Winner/s who can’t make it during the awarding ceremonies must be taken full video by the district supervisor with a simple message which will be played during the awarding.
Authorized member of the family may represent the honoree to receive the token of appreciation.
