answered and have received a copy of this form for you to keep.

RESPONDENTS’ RIGHTS, BENEFITS, AND RISKS

You may withdraw your consent any time and discontinue participation without penalty.
You are not waiving any legal claims, rights or remedies because of your participation in this
research. If you have any questions about your rights as a study subject or you would like to
speak with someone independent of the research to obtain answers to questions about the
research, you have the right to do so.
You will not directly benefit from the research. However, the researcher may give first
hand information on the importance of information literacy skills to you.

Your participation is voluntary. There are no anticipated risks or benefits to your

participation. The study is meant to gauge the level of your information literacy skills.

DATA –GATHERING PROCEDURE.

You will be asked to answer an Information Literacy Skills questionnaire by the
researcher. You will be reached by the researcher in a place of your preference. You will be
asked some questions regarding your study and reading skills to supplement what is asked in the
questionnaire.

COMPENSATION
You will not be given any compensation or incentive from the research.
