(I)-Attested Educational Certificates from Guzzeted office (Only Governmental

Recognized School, certificates from Bard of Intermediate and Secondary education
will be acceptable.)
(II)-Character Certificate from the head master/principal of most recent school/collage.
(III) Four attested passport size photographs from the Guzzeted officer. Paste one
photograph on the specified place given on the form.
