PUBLIC NOTICE
PREPARATION OF DOCUMENTS FOR
MBBS/BDS ADMISSIONS, SESSION 2022-23
The admission process for MBBS/BDS programme in Government Medical & Dental Institutions of
the Punjab will begin after the declaration of official result of Medical and Dental College Admission
Test (MDCAT)-2022 by the Pakistan Medical Commission (PMC). The Admission Notice shall be
advertised separately in accordance with the schedule to be announced by the PMC. .

The aspiring candidates should know that with their online applications, they shall have to upload
and submit the following MANDATORY documents without which their application shall be
considered incomplete and shall be rejected. The list of these mandatory documents is being
provided here for the convenience of the candidates. They are advised to prepare a PDF copy of
these documents well in advance using a mobile device or scanner.

Please note that UHS shall invite applications for and make admissions
on the following categories of seats:
1. Open Merit
2. Students with Disabilities
3. Under-Developed Districts and Cholistan
4. Overseas Pakistanis
5. Reciprocal( Merit seats for Punjab-Domiciled Students in other
Provinces/Regions)

MANDATORY DOCUMENTS FOR ALL CATEGORIES OF SEATS

i. Matriculation (SSC) certificate or result card issued by concerned BISE/Equivalence Certificate
of O-Level, etc., issued by the IBCC;
ii. HSSC Pre-Medical result issued by concerned BISE /Equivalence Certificate of foreign
examination (A-Level, IB, American 12th Grade, etc.) issued by the IBCC. The candidates having
foreign qualification shall also upload their original results issued by the foreign
university/board/school verifying their subjects of study (e.g. Biology, Chemistry,
Physics/Mathematics);
iii. Domicile certificate of the candidate. Domicile of Punjab is a mandatory requirement for
admission on aforementioned categories of seats. However, candidates having domicile of
Islamabad (ICT) can apply on Open Merit seats only. In case of Overseas Pakistanis, Punjab
domicile of Father/Mother shall also be acceptable for candidates born outside Pakistan);
iv. Valid CNIC/POC/B-Form/Passport of the candidate;
v. Valid CNIC/POC/B-Form/Passport of Father / Mother/Guardian of the candidate;
vi. Recent color photograph of the candidate taken in a white or sky blue background;
vii. Disability Certificate issued by the National Commission for Rehabilitation of Special People of
Pakistan (NCRDP), Ministry of Social Welfare OR Provincial or District Social Welfare
Department (For Students with Disabilities only);
viii. School Leaving Certificates/Provisional Certificates issued by the Heads/Principals of concerned
educational institutions (Schools & Colleges) of the under-developed district verifying that the
applicant has studied from class one to twelve from the same district. The candidate will also be
required to upload a Certificate (as per template given on UHS website: www.uhs.edu.pk) duly
verified by the concerned CEO (District Education Authority), DEO (Colleges) and Deputy
Commissioner of relevant district (For candidates applying on Under-Developed Districts’ Seats);
 ix. Original certificate issued by Cholistan Development Authority clearly stating that the candidate
is a Cholistani by birth and has actual residence in Cholistan as per template given on UHS
website: www.uhs.edu.pk (For candidates applying on Cholistan Seat);
x. Valid Stay Card / Residence Permit Card / Green Card/ Sojourn/ Resident Card/Iqama or similar
document of the candidate issued by country of residence along with valid Pakistani Passport
OR valid Foreign Passport of the candidate (For candidates applying on Overseas Pakistanis
Seats);
xi. Hifz-e-Quran Certificate issued by a registered Madrassa (For candidates claiming to be Huffaz-
e-Quran); and
xii. Specimen signatures and left thumb impression (LTI) of the candidate in blue/black ink on a
white paper;

PLEASE NOTE THAT:

1. Once the admissions are opened, you will be required to upload and submit all mandatory
documents otherwise your application will be considered incomplete and rejected. If you
apply for more than one category, you must upload additional mandatory documents for
that category as mentioned above.
2. You are advised to scan your documents in black and white at a resolution of 300 DPI. File
size should not exceed 2.0 MB per document. Ensure that all information on the scanned
document is easily readable. If a document is in a language other than English (e.g. Arabic,
French, German, etc.), a certified English translation may be included.
3. You are advised not to upload all your documents as one file. Create one PDF with proper
label for each type of document you are going to upload. However, you must make a single
file of one type of documents (e.g. passport and Iqama, school leaving and provisional
certificates along with the Certificate verified by the district education authorities and the
DC, etc.) or double-sided documents (e.g. CNIC, Passport, etc.).
4. Don’t deform the photograph or signatures by shrinking the width (cone head effect).

CAUTION: It shall be the sole responsibility of the candidate to upload and

submit correct and complete documents with his/her online application once the
admissions are opened. The applicants who are found to have submitted
forged/fake/fabricated documents shall be debarred from admission as per the
the provisions of the Prospectus.
