Question 2 (a)
After careful analysing and investigating the recent and past incidents it was revealed
that the safety system need improving. Management's hiring criteria are unacceptable
because they have hired young drivers that are more at risk because they are not mature
enough to recognise the risks of their work and do not have the necessary experience.
As in the recent accident, he was using a cell phone while driving, which is also unsafe
behaviour.
Past accidents revealed that the young driver did not pay attention to an oil spill and past
over it that cause the forklift to skid. Past accidents also revealed that the goods
transported by the forklift were not well secured as the load fell down.
Another issue observed is that the workers are not doing a good housekeeping as aisles
are obstructed and disturbing the walkways. In the same time the pedestrians that were
avoiding the obstruction did not paid attention to the passing forklifts.
All these incidents were not reported in the writing form only informal complains where
formulated so no evidence was found regarding the incidents and investigations.
Spillages were not cleaned in due time.

Voluntary feedback was provided the workers complained many times to the
management about the unsafe working conditions but the management did not take the
complains seriously and no actions were taken in resolving the issues. Managers are not
visiting the warehouse on a regular basis showing lack of interest in improving the health
and safety standards. There are no employee safety and health representatives to whom
employees can turn for advice and to help them raise safety and health issues and
concerns.
The warehouse manager told me that no allocation of resources has been made for
improving health and safety. The manager informed me that he has no problem to
allocate budged to improve health and safety, if it will not cause to much disruption to
business.

Investigating the accident in the warehouse revealed the fallowing negative factors that
are affecting the health and safety culture.
The management does not show a strong safety leadership and interest in improving or
evaluating safety standards, as workers complained that managers rarely visit the
workplace. Management prioritises smooth business operations and is reluctant to
budget for health and safety improvements for fear of business disruption showing a lack
of commitment to safety. In the same time, they don’t implement
procedures/management system as the accidents are not reported in writing, and the
informal complains are not fallowed up and investigated. The high number of near misses
and accidents is the main indicator of poor safety culture. The increased number of
accidents is evident as workers indicated that such accidents are frequent. No controls
are implemented to prevent reoccurrence of incidents, as the incident described in the
scenario is obviously repeated and several similar accidents or near misses have already
occurred.
The wrong housekeeping by placing goods on the walkways and oil spills is another
indicator that the health and safety rules are neglected.
A high rate of worker absenteeism indicates that workers are either unwilling or unable to
come in because of work-related illness, injury, fear of being victimized, or other reasons.
A high turnover rate also indicates that workers are not happy in their company and
therefore do not stay long because they do not feel secure.
Workers are not happy with the managers as their complains are not taken seriously by
and they are not consulted on the safety matters, there is no safety representative
thought the workers.
The risk assessments are generic and not suitable and sufficient, which also indicates a
lack of commitment and performance by management.
