TO CHANGE NAME

I, DOMINADOR S. DIZON, of legal age, Filipino and resident of General

Santos City, Philippines, after having sworn to in accordance with law depose and
state:

1. That I am the President of GENERAL SANTOS ACADEMY OF POLOMOLOK,

COTABATO, INC. which is in the process of registration with the Securities
and Exchange Commission.

2. That I, in behalf of said corporation, hereby undertake to change its

corporate name from GENERAL SANTOS ACADEMY OF POLOMOLOK,
COTABATO, INC. to GENERAL SANTOS ACADEMY, INC. in the event another
person, firm or entity has acquired a prior right to use of the said firm name
by virtue of registration with other government agencies or our name is
identical deceptively or confusingly similar to that of any existing
corporation or to other name already protected by law or is patently
deceptive confusing or contrary to existing laws.

3. That this affidavit is executed to attest to the truth of the foregoing and for
whatever legal purpose and intent it may serve.
