NOW INTO COURT, through undersigned counsel, comes <<Matter.Client.Name >>, a

I.

Made defendant herein is << Matter.CustomField.OpposingPartyName >>, a competent

II.

III.

This Court has jurisdiction and venue over this matter because

IV.

As of the date of filing this petition the parties have been living separate and apart

in excess of ONE HUNDRED AND EIGHTY (180) days and petitioner desires to be divorced;

VI.

Neither party is an active member of the United States Armed Forces or another of its

allies.

VII.
 The parties did not contract or convert their marriage into a covenant marriage.

VIII.

Petitioner prays that the community of acquets and gains heretofore existing between the

parties be terminated as of the date of the filing of the petition herein.

IX.

After divorce, petitioner desires to return to her maiden name of

WHEREFORE, petitioner prays that the defendant be served with citation and a certified

copy of this Petition and that after all legal delays in due proceedings had, there be judgment

herein in favor of petitioner, << Matter.Client.Name >> and against defendant, <<

Matter.CustomField.OpposingPartyName >>, decreeing a divorce a vinculo matrimonii and

terminating the community of acquets and gains heretofore existing between the parties as of the

date of the filing of this petition and ordering that the petitioner may return to the use of her
