WHEREAS, The LESSE is a Philippine Economic Zone Authority (PEZA) registered corporation
engaged in providing Information Technology (IT) enabled Business Processor Outsourcing (BPO)
services to local and foreign clients.

WHEREAS, The principal Requires Transportation Services for its Employee and /or equipment.

TERMS AND CONDIOTIONS:

1. DURATION
That terms of this contract shall commence on 1st. day of August 2021 and will end
on 31st. day of December 2021. Private Vehicle Lease Contract defined under
LTFRB MEMORANDUM ORDER CIRCULAR NO 2021-032 (EXTENTION OF
EFFECTIVITY OF MEMORANDUM CIRCULAR NO. 2021-052 OTHERWISE
KNOWN AS INTERIM GUIDELINES EXEMPTING LEASED VEHICLES FROM
SECURING CPC FOR THE DORATION OF MGCQ AND GCQ)

2. SCOPE OF TRANSPORT SERVICE From 6AM to 6 AM Daily Pick up / Drop Off

employees from home to office, vice versa, as indicted by LESSEE.
3.DUTIES AND RESPOSIBILITIES

a. That the LESSSOR.

a.1 Ensure the roadworthy and safety, good running condition of vehicle.

a.2 With comprehensive insurance which cover injuries caused to passenger / goods
and third parties.

a.3 With current registration required by the LAND TRANSPOTATION OFFICE

(LTO)

b. That LESSEE,

b.1 The vehicle shall be exclusive use and driven by LESSEE or its representative to
shuttle its employee / equipment.

b.2 Should install Covid 19 prevention devices (plastic barriers, seat designation with
physical distancing required by GOVT. agencies).

b.3 Regularly disinfect the above vehicle before and after every trip to prevent spread
of Covid 19 and other diseases.

4.CONTRACT PRICE
