Several forms of questionnaires
1.Guided-Response Type – the
respondent is guided in making his/her
reply
2.Recall type – ex: age, name, sex, civil
status, etc

respondents to recognize a figure, a
word, etc.

purpose on answering the
questionnaire if to
evaluate/rate something .ex.
Attitude towards mathematics.
3.Empirical Observation Method –
commonly used in psychological and
anthropological studies. The data is
obtained through hearing, seeing,
testing, touching and smelling.
4.Test Method – widely used in
psychological research and
psychiatry. Standard test are used
because of their validity, reliability and
usability. Ex. Aptitude test, IQ test
5. Registration Method – data that are
obtained from the NSO, LTO, Dep Ed,
CHED, and other government agencies
6. Mechanical Devices – the use of
technology or mechanical devices such
as camera, projector, videotape, tape
recorder, etc. In chemical, biological
and medical researches, the common
devices are X-ray, microscope,
ultrasound, weighing scales, CT scan,
etc.
