Greetings!

We wish to inform your good office that National Labor Relations Commission Regional
Arbitration Branch IX has recently transferred to a new office building. To this effect, it is the
intention of our office to transfer our Land Bank of the Philippines (LBP) MDS Account and
United Coconut Planters Bank (UCPB) Treasury Account to LBP WMSU Extension Office which is
nearest to our new office building.

Hence, we would like to seek for your express grant to allow us to proceed with the closure of
our LBP MDS Account in LBP Main Branch effective July 1, 2016 and our Treasury Account from
United Coconut Planters Bank effective immediately. That way, we may be able to proceed with
opening of new bank accounts in Land Bank of the Philippines WMSU Extension Office.

A copy of our written correspondence is hereby attached stating approval granted by our NLRC
Chairman, Mr. Gerardo C. Nograles for transfer of our depository/ servicing banks nearest our
office for purposes of convenience and ease.
