SUBJECT:- PAYMENT OF PENSION, DCRG/CVP/FAMILY PENSION IN RESPECT OF SH/SMT/MS

Dear Sir/Madam,
             Sanction for the payment of pensionary award in your favour has been received in this office. You are
advised to visit this office on --. Please bring the following documents with you:

1. Discharge Certificate/Identity Card (Issued by Zila Sainik Board).

2. Intimation letter addressed to you by P.C.D.A. (P), Allahabad/C.D.A. (NAVY) Mumbai/ Dy.C.D.A. (AF)
Delhi Cantt Record Office/Head of the Office.

3. Copy of PAN Card duly attested by a Gazetted Officer/Zila sainik welfare Officer.

4. Copy of AADHAR Card duly attested by a Gazetted Officer/Zila sainik welfare Officer.

5. A consolidated certificate duly filled up (form enclosed) certifying:

    a. Non-employment/re-employment.
    b. Non-drawal of any other pension.
    c. Non- counting of previous service.
    d. Non-marriage/ non- remarriage by family pensioners attested by Village
        Pradhan or responsible officer.

6. In case of re- employment in Central Govt./State Govt/Public Sector Undertaking/Bank etc a certificate
singed by the re- employer in the format enclosed.

7. Copies of photograph (joint photographs in case of Joint Notification cases) duly attested by a Gazetted
Officer/Zila sainik welfare Officer.

8 (a).NBS Option from (Copy enclosed) duly completed and signed by the Manager of one of the NBS bank
branches mentioned on the reverse of this letter. (It may be noted that only single account is opened).
   (b).Cancelled cheque/Bank Account Statement/Copy of Bank passbook of the saving bank account in which
Pension is to be drawn.

9. Two witnesses drawing pension from this office.

10. Three Revenue Stamps.

11. Medical Allowance.
