This is RESTY B. CASACLANG, School Head of Bantayan Elementary School,

Lingayen, Pangasinan. 

We are writing to you to signify our intent as a school to participate in the opening of
limited face to face or in person classes in BANTAYAN ELEMENTARY SCHOOL for
the current school year.

*We are humbly asking your team for an inspection signifying our readiness for face
to face classes.

If accepted, please be rest assured that we intend to: 

 Maintain the requirements set under the Schools Safety Assessment
Tool. 
 Maintain our facilities (handwashing, classrooms) to guard the safe of
school children who will participate in the limited face to face.
 Ensure the wearing of facemask and social distancing at all times for
learners, teachers, non-teaching personnel and parents who are in the
school.
 Mobilize of contingency plans and referral systems.
