On 17 /10 /2022 at female surgery department unit(1), which

consist of 5 rooms each room has 4 beds, there are 8 patients on the unit

today distributed as following No. of admission (3) - No. of pre operation

(1) - No. of discharge (4), 1st patient: named Noah Ebrahiem Ali, 28

years, married, Muslim, entered hospital 16 /10 /2022, room number (1),

bed number (2), diagnosed as cholecystitis and prepare for operation,

conscious, GCS 14/15,pale and fatigue, complained of severe pain at

right side increase by movement and decrease by rest, patient connected

with peripheral cannula (pink color) at right arm. nurse named Amal El-

Sayed Ali checked vital signs (T:38,5, P:80, BP:110/60, R: 20), the

doctor named Mohamed Ali Hammed ordered Abdominal ultra-sound

and (CBC, PT ,PTT ,INR) are done, but results not arrived yet and the

doctor ordered to observe the patient for any complication and give the

following medications and the same nurse gave it all at 10 am:

On 17 /10 /2022 at Medical department unit (1), which inculde 12

patients, distributed as following No. of admission (2) - No. of post

operation (5) - No. of discharge (5) and it's 19 bed capacity, there is a

patient named Mona Ahmed Mohamed 55 years, entered hospital on 17

/10 /2022 at 9:00am, room number (2), bed number (1), Conscious,

fatigue, pale with diagnosis (liver cirhosis ) complained of sever

abdominal pain not radiated increased with movement and decrease with

rest. At admission time patient connected with peripheral cannula (blue

color) at right arm, The nurse checked vital signs at 9:30am (T: 37, P: 72

BP: 110/80, R: 20), the doctor named Osama Ibrahem Mohammed and

ordered investigation Abdominal ultra-sound, (CBC, PT , PTT ,ESR) are

done and results arrived on the same day and it was normal and the doctor

ordered the following medications and the same nurse gave it all at 10 am

On 17 /10 /2022 at pediatric department unit (2) which inculde 5patients,

distributed as following No. of admission (2) - No. of discharge (3)- and

it's 18 bed capacity, there is a patient named samah Ahmed Mohamed, 10

years, entered hospital on 14/10 /2022 at 9:00 am, room number (3), bed

number (5), Conscious, fatigue, pale with diagnosis (Tonsilitis) .preparing

for discharge .it was complaine of fever, difficult in swallowing , and

moderate abdominal pain radiate at whole of the body increase with

swallowing and decrease with rest. The nurse checked vital signs at 9:30

am (T: 39.2, P: 75, BP: 110/80, R: 25), pt connected with a peripheral

cannula (yellow color) at right arm, the doctor ordered the following

medications and the same nurse gave it all at 10:00 am.
