PROGRAM IMPLEMENTATION PROPOSAL

PROGRAM TITLE: specify your Program title

I. PROGRAM PURPOSE AND RATIONALE

A. BACKGROUND
 Why is there a need to do this program/project/intervention? What is its importance?
 Explain how the idea for program originated, relating this (if possible) to the organization’s mission,
program vision, and/or population needs.
 Make sure to support this presentation with research literature where possible.

B. PROGRAM CONTEXT
1. Describe the context of the program. This should include the target population – its
characteristics; the setting – important general information to describe setting of your program
implementation.

II. GENERAL OBJECTIVE/S

 What is the general aim/objective of the program?


 What do you want to accomplish after the implementation of the program/intervention?

Specific Objectives:

 State clearly each of your specific program objectives/goals.


 If possible, state the intended outcome for each objective/goal.
 Guide questions:
o Who will this program impact and how?
o How will the program improve the operations and effectiveness of the service delivery of the
institution/organization/hospital/office?

III. PLAN of PROGRAM IMPLEMENTATION

 COMPREHENSIVELY discuss here each step undertaken in implementing your program/intervention.

A. Target Participants (specify the population size, describe the number of target participants)
B. Program Setting (describe the location of the program implementation, the physical set-up/space/scenario of the
setting of the program implementation)

C. Resource Allocation (Describe the program’s resource capacity, providing details of the total human resources
(e.g., staffing) and financial budget supporting the program)
o For the Human resources – specify and describe each of the team member’s roles, tasks and responsibility
in the program implementation
o You may present this in tabular form

D. Program Implementation Activities


o Describe the step by step process of the planned implementation of the program/project/intervention
o Include the schedule of activities – include tentative dates as well as the specific activities included in your
program implementation
o This may be done in tabular form and should include the following in the columns (Date & Time, Activity,
Person In-charge, Resources Needed)

E. Program Evaluation
o Clearly describe the evaluation tool to be used to assess or evaluate the effectiveness of the program You
may adapt an existing Evaluation tool for this
o Include a copy of the Evaluation Tool in the Appendices.
