A pleasant day. This letter is to inform you that we, the students of Batangas State University –
Alangilan Campus, Class IE- 1101, are currently conducting a community-based project and plans on
disseminating survey questionnaires in your locality this November 24, 2022. The aim of the survey to
be conducted is to assess your community status which would greatly contribute when brainstorming
possible projects that could benefit both the community and the students that would conduct the
project.

With this being said, we hereby humbly ask your permission to conduct this survey in your
peaceful community, Nueva Villa, Alangilan, Batangas City. We assure you that the information to be
gathered within the responses to the survey shall be treated with confidentiality and will be used solely
for educational purposes.
