1 Scope and field of application


1.1 Purpose
The purpose of this document is to outline the requirements to ensure that all lifting
activities is carried out safely and efficiently in Medco E&P (Offshore) and in line with
internationally recognized standards within this discipline.

1.2 Scope
BU Lifting document framework consisting of 15 documents pertaining to rigging, lifting
& mechanical handling. This includes competency in maintenance, use, inspection and
certification of lifting appliances and accessories and competency in execution of lifting
operations.

If there are lifting procedures conflicting with this standard, the requirement in this
standard still prevails.

All personnel working in Medco E&P (Offshore) must comply with all of these
documents, and thereby ensuring risks associated with lifting and mechanical handling
are managed within acceptable limits, supporting the ALARP principle.

Lifting activity beyond the principle and philosophies of the above reference shall
proceed with the consultation to Lifting Technical Authority.

1.3 Field of Application


This document shall apply to all Medco E&P (Offshore) owned lifting gear and all
lifting/handling activities in offshore including logistics. It includes maintenance,
competency, inspection, certification and execution of all aspects of rigging, lifting and
mechanical handling. The document shall also apply to all contractors of Medco E&P
(Offshore) who intend to carry out lifting activities on any Medco E&P (Offshore)
asset/facility.

3 Management responsibilities

Shall be accountable for the Medco E&P (Offshore) Rigging, Lifting and Mechanical
Handling Standard.

 Senior Managers or their delegates


Shall be responsible for the implementation of the Standard on Medco E&P (Offshore)
assets/facilities under their control.

 Sr. Manager HSE


Shall be responsible for the update of this Standard to be in line with the law and
regulations of the Republic of Indonesia.

 Technical Authority for crane and lifting


Shall be responsible for the technical content of the standard and its associated
procedures and guidelines as well as advising on the application of the Standard and
procedures/guidelines in practice.

4 Definition
Competent person
Someone who is capable of identifying existing and predictable hazards in the surroundings
or working conditions, which are unsanitary, hazardous, or dangerous to employees, and who
has authorization to take prompt corrective measures to eliminate them.

Qualified person
Someone who, by extensive knowledge, training and experience, has successfully
demonstrated his ability to solve or resolve problems relating to the subject matter, the work,
or the project.

Certified person
Someone, who is required by local Indonesia regulation to have certification related with
lifting operations in oil and gas activity.

Safe Working Load (SWL)


The maximum load (as certified by qualified person) which an item of lifting equipment may
raise, lower or suspend under particular service conditions, e.g. the SWL can be lower than
the WLL. The Safe Working Load as determined by qualified person must NEVER be
purposely exceeded

Working Load Limit (WLL)


The maximum load which an item of lifting equipment is designated to raise, lower or
suspend, the WLL does not account for particular service conditions which may affect the

final rating the equipment, The Working Load Limit as determined by the manufacturer must
NEVER be purposely exceeded

Routine Lift
Lifting activity that frequently carried out within daily or weekly basis and within the normal
operating parameter of the Crane/Lifting appliances, e.g. personnel transfer, workshop
overhead lift, or load has known and evaluated weight, shape and center of gravity (same
cargo).

Non routine lift


All non routine lifting activities will be classified into the following three types
 Simple
 Complicated
 Complex

Complex/Critical lift
non-routine crane lift requiring detailed planning and unusual or additional safety precautions.

5 Standard / Guideline
5.1 Equipment Certification
Lifting devices and equipment must have been certified for use within the last 12 months (at a
minimum) or per regulatory requirements, whichever is stricter:
1. All cranes and forklifts shall have a valid Inspection Company test certificate.
2. All cranes and forklifts shall be subjected to annual inspection by an Independent Third
Party.
3. All lifting gears shall be certified in accordance with the government regulation (cross
referenced with Rigging Equipment Management Guideline – ID-G-BU-00000-00000-
00315) and validity certificate is 6 months.

5.2 Personnel Competency


Personnel competency requirement is defined based on his/her roles and ability to take
immediate action to identify and remove hazard during certain time of lifting operation.

Any personnel at worksite and directly involved in and working with lifting equipment
shall have sufficient lifting competency based on work roles AND shall have valid lifting
certificate from Migas/BNSP.

Any personnel involved in preparation, review, and approve lifting activities who are not at
worksite and does not have ability to immediately removing hazard DO NOT NEED to
have lifting certificate from Migas/BNSP BUT they DO NEED to have sufficient knowledge
to prepare, review and approve the plan/permit. 

However, when this person being at worksite and directly involves in and working with
lifting equipment, this person DOES NEED to have Migas/BNSP certificate.

Any personnel at worksite, not directly involved in and not working with lifting
equipment, but have the ability to immediately removing hazard, DOES NOT NEED to have
Migas/BNSP certificate.

5.3 Pre-Lifting Execution Requirements


1. A qualified person has assessed the lift method and equipment.
2. A competent person has conducted visual inspection of lifting devices and equipment,
including rigging of the load.
3. Critical lifts have undergone a risk assessment, and a Lift Plan has been approved.

5.4 Safety Requirements


1. All cranes shall be fitted with safety systems in accordance with regulations, codes and
standards (i.e. anti two block, limit switch, audible reversing alarm, safety latch on
hook, etc).
2. All rigging equipment shall:
• Have relevant certification documents and be certified for use
• Be inspected and load-tested as appropriate as per Medco E&P (Offshore)
approved guideline listed below prior to being put into service.
• Be visually inspected each time prior to use,
• Be used in accordance with their design
• Be maintained in good condition for safe use.
• Be color coded
• Be clearly and legibly marked on all lifting devices and equipment load limits. Lifting
activity with the use of any structure that receives load with no SWL marked or
information such as scaffolding, beam, pipe, etc. shall be properly assessed and
calculated, and then reviewed & approved by Medco E&P (Offshore) Lifting
Technical Authority and Medco E&P (Offshore) Structure Technical Authority.

3. Lifting Equipment operator shall:


 Ensure that lifting equipment is in proper conditions & its position is on a solid
and flat surface.
 Ensure that the safety devices (e.g. outrigger) on lifting equipment are properly
installed.
 Ensure that maximum load limit are available and clearly read & the load does
not exceed the lifting capacity.
 Ensure safety devices installed in lifting equipment are functioning properly,
prior to lifting.
 Ensure that there are trained and qualified personnel to supervise rigging
activities.
 Understand the hazard of “line of fire” and evacuation route when the load
moves to the wrong direction.
 Ensure to get a clear signal from spotter/signalman when lifting process is
unseen (blind lift)

4. Supervisor shall:
 Ensure that lifting plan is in place and work permit has been approved.
 Ensure that the certificate of lifting operator and rigger is still valid.
 Ensure that the lifting equipment and hoist are in good condition and have valid
certificate.
 Ensure that the signal methods and communication are suitable with the
standards and agreed.
 Ensure tag line, push/pull stick bar, or other proper equipment are available to
control lifting load.
 Ensure personnel who direct the load understand “line of fire” hazard and
evacuation route if the load moves to the wrong direction.
 Ensure condition changes (e.g. Weather, wind) have been discussed and
mitigated.

5. Inspection, testing, and maintenance of rigging equipment shall meet the defined
performance standard and condition target.

6. Any non-conforming lifting gear shall be discarded.

5.5 General Lifting Operations Standard


1. The load weight has been verified to not exceed dynamic and/or static capacities of the
lifting equipment.
2. Prior to lifting, the operator has confirmed that safety devices installed on lifting
equipment are operational.
3. Tag lines, poles, or other appropriate means are available as the primary methods to
control lifted loads.
4. The person(s) guiding the load into place understand Line of Fire hazards and have an
identified means of escape, should the load move.
5. Clear communication (including hand signals) is established and can be maintained by
all involved parties during the lifting operations.
6. Hand signals to crane and derrick operators shall be agreed between signal
man/banksman and crane operator prior to the lifting. An agreed illustration of the
signals shall be posted at the job site and visibly maintained.
7. For blind lifts, a competent spotter/signalman uses radio communication and direct
visual contact with the lift operator.
8. For Lifting Operations to commence, barriers have been put in place, including
preventing anyone from walking under suspended loads or into the Line of Fire.
9. Changing conditions (e.g., weather, winds) have been discussed and prepared for.
10. No mobile crane travels with load.
11. Wind speed for lifting activity consideration can be used for guideline in Appendix A.

5.6 Hazard
Typical hazard related with lifting activity may include:
 Dropped object
 Crane failure/collapse
 Foundation instability
 Struck on object (i.e. electrical tower, building/structure)
 Nature hazard (i.e. wind, wave, lightning)

5.7 Issued and Auditable Date


This document is issued as per Issued For Use (IFU) stated date on this document cover
page and will be auditable within 1 month after the issued date.

5.8 Verification & Deviations


The compliance with this standard and subsequent document shall be audited and verified.
Any gaps shall be identified and closed through corrective action plans. Mitigation plans may
be established to meet the ALARP principle. Deviations from the standard or other
procedures may be handled through HSE exceptions as per the already established process.

Appendix A – Lifting Condition


This guidance is applied for lifting:

 From static point to another static point, or


 From static point to dynamic point, or
 From dynamic point, which must be treated to be static first (i.e. moored barge, DP vessel,
small boat 2 point tethered to platform or vessel) to ensure stability of crane on dynamic point.

Crane must be positioned at static point or at dynamic point which has been treated appropriately to be
static.
