I. PURPOSE
To establish and implement a process that provides employees, contractors and
visitors with the required information, knowledge, basic skills to perform any activities
at offshore facilities and/or office building in a safe, professional, and environmentally
manner.
.
II. SCOPE
This Procedure is applied to all employees, contractors and visitors in Medco E&P
Natuna Ltd, Medco Sampang Pty. Ltd., Ophir Madura Offshore Pty. Ltd. (herein later
called as “Company”)

III. DEFINITION AND ABREVIATION

Company: Medco E&P Natuna Ltd., Medco Sampang Pty. Ltd., and/or
Ophir Madura Offshore Pty. Ltd.

Contractors: Non-Company employees performing work for or on behalf


of Company.

Contract Owner: Company employee who is assigned to manage particular


contract and as a focal point to be contacted in regard of the
fulfillment of contract

Dedicated Trainer: A trainer who has been assessed and declared to be


passed to deliver HSE Orientation training materials.

Employees: Personnel who work for Company under a work agreement


and receive payment.

HSE Passport: A tool in form of digital or virtual card to record all relevant
data of personnel (employees, contractors, visitors) such as
personal data, emergency contacts, medical records, MCU
and HSE Orientation records, training and competency
records, mobilization/demobilization, etc.

On-Site Trainer: A dedicated trainer who delivers HSE Orientation training


materials at site.

Post-Test: A written test given to all participants/ trainees who have


completed their initial HSE Orientation training to measure
the understanding and effectiveness the system

PTS: Stands for Personal Transportation System – a flight


reservation system for personnel who are going offshore.

Refresher Test: A written test given to all personnel who have their HSE
Orientation training expired. The result will determine
whether the HSE Orientation refresher training is required.

Sponsor: Company employee who is assigned as a sponsor and act


as a focal point to make an arrangement of the offshore
visits for visitor(s).

IV. ROLES AND RESPONSIBILITY

a. Manager Performance Assurance and Audit (PA & Audit)


Manager PA & Audit in coordination with relevant parties will be responsible to:
a. Assign person(s) as dedicated HSE Orientation Trainer
b. Review and approve the contents of HSE Orientation materials
c. Review and approve the post-test materials
d. Conduct annual review of the effectiveness of HSE Orientation training
implementation

b. HSE Orientation Dedicated Trainer


HSE Orientation Dedicated Trainer shall be responsible to:
a. Develop, review and revise the HSE Orientation materials.

b. Notify & send the revised HSE Orientation materials to the Manager PA &
Audit to obtain approval

c. Develop Post-Test materials to determine whether participant/trainee is


pass or not

d. In coordination with HSE admin to input the updated HSE Orientation


training record into the electronic database system

e. Prepare a monthly report of the implementation of HSE Orientation training.


The report (at a minimum) consists of; number of personnel have been
trained, number of personnel passed/not passed the training, any material
required changes (if any), etc.

c. Contract Owner/ Direct Supervisor/ Sponsor


Contract Owner/ Direct Supervisor/ Sponsor shall be accountable/responsible
to:
a. Ensure the HSE Orientation training of his/her personnel are VALID

b. Register his/her personnel to the scheduled HSE Orientation training


session as required

c. Keep the training record of the HSE Orientation training

V. PROCEDURE
1. All employees, contractors, and visitors who are going to visit offshore fields
(incl. Matak Base), have to get their valid HSE Orientation
2. New employees, contractors and/or visitors who have not received the HSE
Orientation shall undergo the session prior to visiting offshore fields
3. Direct Supervisor/ Contract owner/ Sponsor shall inform and register their
respective employees, contractors, or visitors to the HSE Orientation
Dedicated Trainer
4. A regular (weekly) HSE Orientation training session is scheduled and held
in Jakarta. If the regular class (offline) is unable to be conducted, a virtual/
online class session or other applicable method have to be held as an
alternative
5. HSE Orientation training contents should include at a minimum the
following:
a. Company Policy
b. HSE Management System in brief
c. Life Saving Rules
d. Hazard Identification/ Risk Management (Top 5 major hazards)
e. Electrical Safety (Electronic devices)

f. Relevant critical HSE standards/ procedures/ programs (such as:


Work Permit, Electrical Safety, Process Safety, Stop Work
Temporarily, Safety Card, etc.)
g. Introduction to office building and offshore facilities (includes
applicable site-specific procedures)
h. Emergency Preparedness (e.g. muster points, etc.)
i. Waste Management
j. Ethics and Code of Conduct
k. Sustainability Development
6. All participants/ trainees have to undergo the Post-Test examination to
ensure the understanding of the HSE Orientation materials
7. The result of post-test would reflect the effectiveness of HSE Orientation
training delivery
8. Participants/ trainees who have not passed the post-test, will be given the
second examination within one week since the first test conducted
9. In the event where result of the second examination is not pass, the
participants/ trainees should undergo the full HSE Orientation training in the
next opportunity
10. All data of participants/ trainees (consist of; name of participants/trainees,
PTS ID, date of training, and expiry date of training) who have passed the
post-test will be recorded and inputted into the electronic database system
(e.g. PTS, HSE Passport)
11. The HSE Orientation training is valid for 4 (four) years
12. Personnel who have their HSE Orientation training expired, shall undergo a
refresher test to determine whether or not the refresher training is required
13. Personnel who have not passed the test, shall undergo the refresher HSE
Orientation training

VI. PROCEDURE AND HSE ORIENTATION CONTENTS MAINTENANCE


1. This procedure document will be regularly reviewed as per HSE Document
Procedure
2. The HSE Orientation training materials (includes the Post-Test and the
Refresher Test) shall be reviewed annually along with the reviews of its
effectiveness
3. Any modification on the HSE Orientation training materials shall obtain
approval from Manager Performance Assurance and Audit
