AFFIDAVIT OF UNDERTAKING

I, Company Authorized signatory, a Filipino, of legal age and capacity, after

having duly sworn in accordance with law, hereby depose and state, to wit:

1. That I am the Title of Company Name. with office address_____ at

Street_____ Bldg. no.____ Office no._____, P.O. Box ____, Doha, State
of Qatar;

2. That I undertake to hire and employ the workers to ensure that they will
work in accordance to the function and duties and no to be deployed as
Domestic Helper or do household cleaning.

3. That I likewise UNDERTAKE and ASSUME full and complete

responsibility for all consequences which may arise in connection with
the said registration.

4. That i am executing this affidavit with the requirements for the issuance
of POEA registration in favor of our company.

5. That this Affidavit is executed to attest to the truth of the foregoing and
for whatever legal purpose and intent it may serve.
