DIRECTION: This rating scale is a guide in determining the growth of the student teacher and is
helping to enhance his/her improvement. Please put a check inside the box which you honestly
believe represents the student teacher’s performance in each analysis item.
5- Excellent 4-Very Good 3- Good 2-Fair 1-Poor

1. Dresses appropriately, clean, and well-groomed.
TEACHING PRACTICE
2.1 Voice is clear and easily heard.
2.2 Uses appropriate language and grammar.
2.3 Shows evidence of competency in the skills and knowledge in the
subject area.
2.4 Prepares and makes adequate use of lesson plan.
2.5 Executes lessons well.
2.6 Uses evaluation techniques and instruments; identifies students’
progress; institutes remedial measures.
INSTRUCTIONAL MATERIALS
3. Uses a variety of instructional materials and proper references; Utilizes
appropriate teaching aids and devices and a variety of well-planned
techniques.
TEACHING ATTITUDE/LEARNER’S ENTHUSIAM
4.1 Shows enthusiasm for teaching.
4.2 Enjoys coming to class; feels he/she is learning.
4.3 Participates in school affairs and activities.
STUDENT TEACHER’S PUNCTUALITY
5. Reports on time regularly.

6. Records are neat and adequately kept on each student.
