Subject SPMT Incident Thailand 16 November 2008

Ladies and Gentlemen

On the 16th of November 2008 an incident occurred in Thailand on the Foster Wheeler/Worley Parsons
(FFWP) project. The site is a fabrication yard for modules being built and subsequently sea transported to
the Pluto Project in Australia.

The incident occurred during the transportation of a Pre-Assembled Module (PAR) by means of a single 24
axle line Kamag SPMT trailer, as seen in the photograph below. During the operation the trailer began to lift
in an uncontrolled manner without operator input or control, this caused the trailer to elevate on only one
side of the combination, resulting in the combination and cargo configuration as seen below. The operation
was immediately halted as trailer stability and safe operation was obviously compromised.

It should be emphasised that no personnel were injured and no equipment damage took place. It
should also be noted that the cargo stability was also not compromised.

On completion of realignment and levelling of the combination, the PAR module was positioned on
temporary supports to facilitate safe withdrawal of the combination from the underside of the PAR, this
allowed a replacement combination to be inserted under the PAR for safe onward transportation.

Mammoet immediately deployed a senior technical representative to site to assist in the investigation and to
determine the root cause of the incident. The investigation has identified that the cause of the incident was

attributed to a Phoenix plug located in the computer box on the trailer, which was not sufficiently secured
into its housing causing a poor connection, this in conjunction with stray copper wire particles found under
relay K8, caused a short circuit to this component part which controls all lifting, braking and driving motions.

It should be noted that the manufacturer of the equipment (Kamag) has endorsed our findings and
confirmed this in writing.

1) The following immediate preventative actions shall be implemented to prevent recurrence.

• All relays shall be checked to ensure they are free from contamination
• All Phoenix plugs shall be checked to ensure they are fully engaged
• Trailers thoroughly examined for damage and a full function test conducted
• Relay K8 to be replaced

2) The following long term preventative actions shall be implemented to prevent recurrence.

• Change Phoenix plugs for screw or clip type (periodic checks until replaced) (Approval required
from Kamag prior to replacement)
• Computer boxes shall be cleaned by use of vacuum cleaning methods only and not by use of air
guns. (Reduction in potential for spreading contamination to component parts).
