FACILITY OVERVIEW
Chemunaa Chps Compound is a government facility under the Ghana Health Service, founded in
2022, located at Chorkor-Chemunaa with GPS address (GA-446-7812) and has a minimum bed
capacity of 1.
A. GOVERNANCE, INFRASTRUCTURE AND SAFETY

A.1STAFFING

Practitioner-in-Charge: Facility has a physician assistant as its practitioner in charge.


Practitioner has 6 years post basic qualification experience. He was present during the
visit.

Others: In addition, one extra PA, two RGNs, and one auxiliary nurse as well as MCA
and a lab technician were there to support the in charge.

A.2ORGANIZATIONAL MANAGEMENT
Facility has no mission and vision statement, but has an organogram and patients’ charter
available and on display but not at all the service areas. Few IPC guidelines were on display.
The facility has no inventory for assets available at the facility. No sign of a suggestion box
to take client concern and also clients call nurses on their personal phones to give feedback.

A.3HUMAN RESOURCE MANAGEMENT


Staff were appropriately dressed but had no identification tags with photos. Duty rosters were
available, but not in use periodically. The staff documentation filed were found to be
inadequate. Staff claimed that their documents are not kept at the facility but rather their
main mother hospital. No display of phone numbers of the facility, the in-charge or higher
referral facilities were found. No record of meetings was ascertained.

A.4EQUIPMENT MAINTENANCE
Asset register was available with an MOU on planned maintenance. No logbook to record
faulty equipment was found. All mercury-containing devices are being phased out.

A.5MANAGEMENT INFORMATION SYSTEMS


The facility keeps records of service data. Backup and patient data protection systems were
not in place. EMR migration was in process. The facility reports manually to the DHIMS.

A.6SAFETY AND SECURITY


Structural: Access to the facility is not that easy, a bit spacious, and ramps to aid
children and the disabled to the facility were available. There are no fire extinguishers,
smoke detectors, and no emergency assembly points as well. Floors are neat, with no
cracks and non-slip and odorless. There was no backup to the power supply and no gaps
between the walls

Signage: Directional signs were boldly written within the facility but no warning signs at
all.

A.7WATER, SANITATION & IPC


The facility has adequate water supply and back up water supply, no pedal waste bins were
found at service areas and offices, sharp boxes were in place, no segregation of waste prior to
final disposal, veronica buckets with elbow taps and sanitizers were available, posters on
appropriate hand washing displayed. Instruments and soiled linens are decominated by
dissolving in chlorine solutions. No access to incinerators and septic tanks.

A.8SLUICE ROOM/WASHROOMS

Clients and Inpatient: No separate washrooms for both male and female. Area is clean
and dry. Water closet available was functional and toiletries were present.

Staff: No separate washrooms for male and female as well. Washroom was neat,
toiletries available, no pedal bins and room was odourless.

B. CLINICAL/PUBLIC HEALTH SERVICES

B.1CLINICAL SERVICES
Facility has no basic emergency services but treat minor ailments like malaria, URI among
others, has a dispensary, offer basic laboratory services and an observation room is available.

B.2RECEPTION/REGISTRATION/GENERAL WAITING AREA


Facility has a waiting area. Area is conducive with an excellent setup. Screens and curtains to
enhance privacy were available. An attendance registration book and PPEs were evident as
well. A pedal bin is present but without the appropriate liner. Area is illuminated and well
ventilated.

B.3RESUSCITATION AREA

Equipment: All equipments available with the exception of spatula.


Medication: Essential medication were in place and in appreciable quantities. However,
iodine tincture (antiseptics), dextrose and normal saline were unavailable.

B.4TRIAGE/NURSES BAY
Available and amply equipped with all functional general equipment for triaging. However,
there was no patient trolley and triage color protocol was absent at the area.

B.5CONSULTING ROOMS
Facility’s consulting room which is amply resourced with tendon hammer, hand washing
facility, ophthalmoscope, and tape measure among others.

B.6GENERAL CHANGING ROOM


Changing rooms are well spacious, well ventilated and illuminated but np lockable cabinets
and seats.

B.7TREATMENT/PROCEDURE ROOM
All items in this area are available and in good condition

B.8LABORATORY SERVICES
Facility provides tests for Glucose, Malaria, Pregnancy, Hepatitis B, urinalysis and HIV
screening

B.9OBSERVATIONAL SERVICES
Beds, Screens, Lockers, Hand washing facility are present.

B.10 PUBLIC HEALTH SERVICES


Facility offers public health services but without registers.
