CHECKLIST FOR APPLICATION FOR AMENDMENT OF QUANTITY, WEIGHT, DESCRIPTION


AND OTHER DETAILS

CONSIGNEE:
BL NUMBER:

1. Notarized request with explanation and undertaking from the consignee / shipping line / forwarder, addressed
to the Deputy Collector for Operations, stating valid reasons for amendment (CMO 31-2015);

If the request is made through a representative, the following shall be required:


a) Authorization:
- If CORPORATION – secretary certificate and a copy of General Information Sheet (GIS)
- If SOLE PROPRIETORSHIP – Special Power of Attorney (SPA) and a copy of DTI Certificate of
Registration
b) Valid Identification Document (I.D.) of Consignee / Corporate Secretary and authorized
signatory / representative

2. BOC Official Receipt for amendment fee of Php500;


3. Clearance from Law Division that there are no pending seizure/abandonment proceedings;
4. Clearance from Office of the District Collector that subject b/l is not covered by an alert/hold order;
5. Notarized original copy of Certification from shipping lines with ID of signatory together with both original and
amended bill of lading;
6. Certificate of Registration (accredited by AMO) – consignee.

liable for the wrongful delivery of the shipment;
7. Notarized acceptance of ownership of the new consignee with undertaking that the BOC shall not be liable for
the wrongful delivery of the shipment;
8. Certificate of Registration (Accredited by AMO) – both Consignee.

CONSIGNEE:

1. Notarized request with explanation and undertaking from the consignee / shipping line / forwarder, addressed
to the Deputy Collector for Operations, stating valid reasons for amendment (CMO 31-2015);

If the request is made through a representative, the following shall be required:


a) Authorization:
- If CORPORATION – secretary certificate and a copy of General Information Sheet (GIS)
- If SOLE PROPRIETORSHIP – Special Power of Attorney (SPA) and a copy of DTI Certificate of
Registration
b) Valid Identification Document (I.D.) of Consignee / Corporate Secretary and authorized
