COMMUNITY ACTION PLAN (PROPOSAL)

Prepare and accomplish the following steps in creating your Community Action Plan. The
final paper will be written in English, Bookman Old style, font size 12, and 1.5 in space. This
is to be presented in class during Online Meet per group/team.

I. Develop Guiding Principles for a Community Youth Team


- Organizational Core Values and Advocacy (These are the standards of team
behavior. These guide how things are done in the team, how people in the team
behave, and what practices are observed. These becomes the bases of the
member’s identity.)
- Organizational Mission (This is the team’s reason for existence. The mission
statement is a means to share the goal of a team. It communicates what is
important to and the direction of the team. Also, it projects its important
competencies.)
- Organizational Vision (This is the team’s picture of the future. This is the set
of ideals and principles that the team holds on to. The vision serves as the
inspiration and drive among its members)
- Organization Image (This the unique element or niche of the team;
logo/symbolisms/significant representations)
- Guiding Principles (These are the team’s beliefs and philosophy concerning
what the team does, why it does it, and how it does it)

II. Community Profiling and Needs Assessment


- Identify the selected Community
- Preparatory and Research Phase (Obtain basic information, knowledge, and
understanding of the situation of the community, consultation and
coordination with the selected community, interview and actual data
gathering)
- Needed data are identified below:
o Community Identification and Overview (Its individuality and
uniqueness, First impression, location relative to other places,
landmarks – Mapping and Diagramming)
o Historical Origin (Legend or real history, Early beginnings, Reasons for
living in the community)
o Ecological Frame ( Physical conditions and boundaries, Demography
and Profile of the People, Service Institutions, Transportation,
Communication systems)
o Life Activities (of the members, heads of the family, mothers, children,
leaders, youths, etc.)
o Leadership (Political structure/ hierarchy, existing programs/
projects)
o Problems/Conflict/Challenges (insecurities, security, economics,
health and sanitation, environment, utilities & facilities, Social and
moral problems, social welfare)
o Impact of the World Outside (Inflow of Ideas/knowledge, influences,
migration, products, leisure, food, fashion, etc)
o Goodness/Privileges/Benefits of Local Life (Location/Site,
Peace&Order situation/Security, Availability of Employment,
Community Relationships)
