1. GENERAL

Purpose

The purpose of the MS is to outline the requirement and responsibilities for works carried
out concrete repair works at berth#4 damaged or deteriorated areas in the cope beams
during the hours of darkness or in the areas where artificial lighting is required.

Project Overview & Scope of Works

The project locations and t h e m ain work scope for this permit is as per the
following:

Environment

*All chemicals shall have MSDS & it must be available at site and properly communicated to
workforce as well.

*First aiders & fire fighters are available for nightshift activities

*Waste Materials shall clearly labelled bins for disposal, The bins shall be emptied through
correct, and as per the Qatar Energy approved -GC16103400 Waste management plan.

* The lighting levels in a workplace should be adequate to ensure that workers can work
without risk to their health from poor or inadequate lighting, does not result in excessive
differences in illuminance within an area or between adjacent areas, managing the health
and safety risks from lighting in the workplace includes:

Planning: Set target for improvement by assessing whether the lighting is suitable and safe
for the work being carrying out

Organizing: if lighting is proving to be problem in the workplace, then they should bring this
to the attention of the performing authority (PA), the Performing authority should also
ensure there are competent electrical technician available to deal with any lighting
problem.

Control: Checking on a regular basis and ensuring the lighting levels & arrangements are
maintained. There should be a system in place for the maintenance, replacement &
disposal lamps & luminaires.

Monitoring: Illuminance level requirements for the workplace shall be as per the Qatar
energy requirements/standards, Monitoring lighting (Measured Illuminance in lux)
conditions will ensure these standards have been met.

JHA: When assessing lighting in the workplace, OA&PA must consider whether the
arrangements for workplace lighting are satisfactory or might present a risk to workers,
contractors, or visitors to the premises.

5.1 PREPARATION OF WORKSITE:

a. Gas Test should be conducted prior to work by Authorized Gas Tester (AGT)
b. Personal should wear H2S toxic clip throughout the job & intrinsically safe
mobile to be used.
c. Proper understanding of the job for all personnel involved to be ensured.
d. Foreman and supervisor performing TBT prior the commencement of the work.
e. The material should be lifted in a balanced way so that the activity will be
executed safely.
f. Platform bridge to be mounted securely & All working at height activities suspended if
wind exceed 20 knots & Tide must be monitored for splashing water over berth when power
tools is functioning
g. Spark arrestor/Mesh for all equipment exhaust system (hydrocarbon berth)
h. For Exceeding working hours must have the OA approval
i. Required email permission to deviate the approved MS, JHA & PTW.

5.2 STRUCRURAL STEEL INSTALLATION

• Installation / Erection

 Any Installation and Erection activity shall be in accordance with approved drawing.
Materials lifting activity shall be carried out by an approved Rigger from Fabrication to Site to
avoid damage to materials and property.
 Lifting of painted structural members shall be done with a nonabrasive choker.
Evidence of materials quality inspection documentation such as receiving inspection and
fabrication approval drawing shall be available prior to installation and erection for easy site
references during installation and erection.
 All miscellaneous structural steel fabrication with connection to main beams, columns and
supports shall be as per approved design drawings. Connection installation are as per the
following criteria specified under PIP STS05130 and 12-SAMSS-007.

 Before any erection and installation work commencement, the erector shall check foundations
and other connection points to confirm their location, orientation, elevation, and condition.
 Erection and Shop drawing shall be available during installation to ensure that all parts, sizes,
and fabricated materials is as per approved drawing.
 Any circumstances discovered by the erector that affect progression, performance, or
completion of the erector’s work activities such as discrepancies between the erection/shop
drawing and the delivered steel members, incorrectly fabricated steel members, or incomplete
or unacceptable work of other contractors affecting the erector’s work shall be immediately
reported to JGC/DEC Engineering Department for final disposition.
 Any damage incurred caused during erection or installation shall be reported for disposition.
Corrective measures shall be completed .
 Temporary erection loads or permanent loads during installation shall not be place on any
incomplete portions of the structure being erected unless the erector can demonstrate by
analysis that the contemplated action is safe.
 Loose timbers, metal sheeting, bolt buckles, tools, debris, and temporary scaffolding shall be
kept restrained or removed from work areas. The erector shall be responsible for securing all
equipment and materials within the erector’s care, custody and control during the erection I
installation operation.
 Before and After erection and installation activities is carried out, job site shall maintain a clean
and safe condition environment at all times and shall dispose of, off the premises all crating,
waste materials, and other refuse that has accumulated as a result of the erector’s activities.
Lifting of painted structural members shall be done with nonabrasive choker.
 Erectors and installers shall verify the following crucial activities prior to commencement of any
erection and installation as per PIP STS05130 standard as follows:

• Check and verify the Structural Stability of the installed or erected structures in connections with
temporary bracing adequacy and guy cables requirement to counteract loading imposed during
erection.
• When Setting of Base Plates, erector shall clean the top of bearing surfaces and the bottom of base
plates. The erector shall set and shim column base plates to correct positions, elevations, and locations
as shown on the erection drawing. If setting nuts are used, they shall be loosened before grouting.
• Check bolted connections in line with high strength bolt tensioning method using calibrated torque
wrenches and approved torque value table.
• All welded connections must be as per approved.
• Any kind of error corrections on fabrication during assembly and erection shall be approved by
JGC/DEC Engineering.

7. HEALTH, SAFETY & ENVIRONMENT

i. All personnel shall wear proper personal equipment (PPE) in
accordance with the Qatar Energy regulation and Health Safety &
Environmental Plan.
ii. The waste management plan must be followed.
iii. All workers will be made aware of safety requirements related to all
activities through toolbox and Safety talks.
iv. All Required work permit will be acquired and kept available at
perspective work sites hanged clearly in plastic folders.
v. Barricade the working areas and use signage’s avoiding non authorized
personnel to enter.

9.PTW SYSTEM

 The purpose of this procedure is to define requirements for the safe

operation of the Qatar Energy Permit to Work System (PTW). The PTW System is
designed to control certain hazardous activities and ensure a safe working
environment so that work carried out is conducted in a safe, coordinated, and
consistent manner.
 Authorized PTW receiver for Qatar Energy / MIC CPW.
 PTW request 48 hours before the task to get approved.
 Approved JHA and Method Statement must be attached to PTW
 PTW must be specific, accurate and valid.

Incident Observer Responsibilities:

Immediate notification of Emergency to Vessel Traffic Control (VTC) VHF on 11 or

MIC General Emergency Number (ERCC) -135 and to Onsite Client
Representative.

Provide the following (METHANE) information:

METHANE is an acronym used to easily remind emergency responders on the

steps to be followed when communicating/reporting back information to Area
Supervisor/Control Room/Fire Station on the incident they are involved with.
