(4) Transfer Certificate and Conduct Certificate issued by the Head of the Institution last studied.

(5) Certificates in support of claim for admission under Special Quota (PWD, ESM, FF, MSP,RURAL) if applicable.

(6) Recent passport size photograph. Copy of CENTAC application form and allotment order.

Important:

1. The allotment of seat is purely provisional. Allotment to a candidate who does not possess the required qualification will
be CANCELLED, if it has been made either based on the FALSE data/certificate or by oversight.
2. If the candidate does not join the course offered on or before the date indicated above he/she shall forfiet the seat
alloted and he/she cannot claim the same seat/allotment subsequently.
