Brief Incident Description

Mr. l Hlope the Security Supervisor was travelling in


LDV #CORS 17, on the Witcons Siding district road
when he lost control of the vehicle in muddy conditions.
The LDV made contact with a berm on the left side of
the district road, causing the LDV to roll over and land
back on its wheels. No injuries were sustained by the
occupants. Based on the dashcam footage, the
passenger was observed not wearing his seatbelt at
the time of the incident.

Consequences

Autocam system notified the standby Security Manager
Security Management attended the scene
Mine Safety officials informed
Management was informed
Reported to SAPS Ogies
