This is to allow/permit our son/daughter, ,a

student, to participate in the Paskuhan 2022, with the theme: “Enlightening Minds, Igniting Joy,
Illuminating Hope” which will be held on December 3-15, 2022. We believe that his/her participation will
be of great help in accomplishing the following objectives:
1. to establish the Paulinian spirit and inculcate camaraderie within the Paulinian Community;
2. to let the students enjoy and showcase their talents and skills through participating in various
fields and activities; and
3. to celebrate and make the students feel the essence of Christmas;
We understand the benefits that our son/daughter will derive from the activities; hence, we are giving this
letter of permission. We free the school from any liability for any harm that may be caused by untoward
incidents during the activities. We will ensure that our son/daughter shall observe faithfully the guidelines
for the activities and shall be responsible for any infractions thereof. Below are the activities participated

LIABILITY WAIVER

voluntarily agree to participate in the Paskuhan 2022, with the theme: “Enlightening Minds, Igniting Joy,
Illuminating Hope” which will be held on December 3-15, 2022 under the following terms and conditions:
1. that I shall abide by the rules and regulations of the University and shall comply with those
imposed for the activities, otherwise I shall be excluded from further participation;
2. that I shall exercise care and diligence in any task assigned to me;
3. that I shall renounce and waive my claim against the school/,my organization/
industry/establishment for injury I suffer, particularly in the performance of my duties and
functions while in the activity; and
4. that I shall be made answerable for any and all liabilities for damages to property or injury to
third person which may be occasioned by my intentional and negligent acts while in the course of
my training.
