- The patient stated the year is 1952.
7 am - 3 PM shift
O- the patient is weak, his face is flushed, his skin is warm and dry.VS
2 PM of BP 99/60 mmHg, Ht170.2cm, HR92bpm and regular Wt 60kg, RR 28
bpm , SpO2 93% on Room Air, Temp 38.9°C

A: the ED ordered Nasal Cannula to keep SpO2>92%, 1L Normal Saline

bolus IVx1,now, 1,500 mg Vancomycin IVPBx1dose,now; initiate the fluid
bolus; monitored VS and recorded. VS BP 86/50 mmHg MAP62 mmHg,
HR 108 bpm , Temp 39.3°C, RR 36 bpm SpO2 88% on Room Air. And
antibiotics are initiated with in an hour of arrival.

R: The patient was transferred to ICU with the VS Art. Line BP 82/48
mmHg MAP58 mmHg
HR 122 bpm, CVP4 mmHg, RR32bpm SVR 640 dynes/sec/m-5, SpO2
90% on Room Air.
