Dear Sir,

This letter is a formal request for a five-day leave of absence without pay, so I may attend the
final coaching for my upcoming Licensure Examination for Professional Teachers. The said event
will be held in General Santos City. My dates of travel are from September 26, 2022 to
September 30, 2022

Attached herewith is my Notice of Admission (NOA) for the October 2, 2022 LET and the
activities to be taken by my Practical Research 2 students for the whole week. My General
Mathematics subject will be relieved by my co-teacher, Rinbert B. Gesta.

I kindly request you to grant me the leave for the said dates. I will resume work thereafter.

Monday

A. Define the following terms:

1. Research Topic
2. Research Problem
3. Research Question
4. Research Hypothesis

B. In your field of Specialization or Strand, enumerate at least 10 timely relevant problems which you
think need an immediate solution.

Tuesday

Proceed to your permanent group in Practical Research 2. After having enumerated the problems
related to your field of specialization, discuss with your groupmates each of the problem and try to
come up with 10 problems which is common or voted by majority of the group to be considered as
potential research problem.

Wednesday

Collaborate with your groupmates. Finalize what research problem you are interested to study. Make a
Research Title out of that Problem.

Thursday

Make a reflection paper on why you choose that problem over the many problems you have
enumerated.
